    I am writing to formally dispute the account listed under account number ********************** - ********, which appears on my credit report. This account was opened without my knowledge or authorization and is the result of fraudulent activity. I have no recollection of applying for or entering into any agreement with CAINE & WEINER.Pursuant to the Fair Debt Collection Practices Act (15 U.S.C. 1692g), I am requesting full validation of this alleged debt. To date, I have not received any documentationsuch as a signed agreementdemonstrating that I consented to or am legally responsible for this account. Without such evidence, there is no legitimate basis for this account to be associated with my name.Because this account was fraudulently established, I request its immediate removal from my credit report and that all collection efforts cease. If you are unable to provide the required validation within 30 days of receiving this notice, you are legally obligated to delete the account from all credit records and discontinue further collection activity.Please confirm receipt of this letter in writing and advise me of any next steps you intend to take to resolve this matter.

    We have completed our investigation of this complaint and closed this account as fraud. We will send a request to the credit reporting agencies to have this account removed from the consumers credit report. This request may take up to 45 days to be reflected on the consumers credit report.

    To Whom it may concern,I am writing to express my deep concern regarding several unauthorized account with **********************/PROGRESSIVE Account number: ******** Account balance: $385.00, which I have been unable to resolve directly with them. I have discovered that accounts have been opened in my name without my knowledge, consent, or authorization. This fraudulent activity is not only a violation of my privacy, but it has also caused significant distress. I expect immediate action to rectify this issue. I am requesting that the Better Business Bureau assist in removing this account from my credit reports.I have attached copies of relevant documents, including my driver's license proof of address, copy of my social security card a cover letter and the my credit report, to support my case.I kindly ask for your assistance in facilitating a resolution to this matter. Please let me know if you require any further details or documentation.Thank you for your attention to this complaint. I look forward to your response and to a prompt resolution.

    To Whom it may concern,This letter is to inform you that I recently received a copy of my credit report that your company publishes and after reviewing it I found a number of items on the report that are inaccurate. The accounts in question are listed below. Please send me copies of the documents that you have in your files as of this date that you used to verify the accuracy of the accounts listed below. COMPANY NAME: CAINE & WEINER ACCT #: ***. ******** $0.00/$136.00 Under the Fair Credit Reporting Act, 15 U.S.C. 1681g I have the right to demand that you disclose to me all of the documents that you have recorded and retained in your file at the time of this request concerning the accounts that you are reporting in my credit report. Please dont respond to my request by saying that these accounts have been verified. Send me copies of the documents that you have in your files that were used to verify them. If you do not have any documentation in your files to verify the accuracy of these disputed accounts then please delete them immediately as required under Section 611(a)(5)(A)(i). By publishing these inaccurate and unverified items on my credit report and distributing them to 3rd parties you are damaging my reputation and credit worthiness.

    I am a victim of identity theft as stated in law 15UFC 1681B you have to give permission for someone to use your info in a credit report also 15 UFC 1681C two. The creditor or credit agency should not furnish any type of info on an account that has been open due to identity theft
