Aspire provides recordkeeping services for retirement plans and accounts. For Aspire to process a request for a distribution from an account, the request must include all required information as well as required approvals from the retirement plan sponsor and/or its third party administrator.
Aspire has reviewed its records related to Ms. *****'s account and determined that, in each instance when Ms. ***** submitted a distribution request, the request was not in good order. In each instance, Aspire promptly contacted Ms. ***** to explain any missing information and/or approvals and to requested that Ms. ***** resubmit all request paperwork (including any missing information and/or approvals) for processing.
Aspire's records show that a distribution was processed from Ms. *****'s account on May 3, 2022.
