                                                                              NOTICE OF CANCELLATION FROM ACTUAL CONTRACT

YOU MAY CANCEL THIS TRANSACTION WITHOUT ANY PENALTY OF OBLIGATION WITHING THREE BUSINESS DAYS FROM THE ABOVE DATE.

TO CANCEL THE TRANSACTION, MAIL OR DELIVER A SIGNED AND DATED COPY OF THE CANCELLATION NOTICE OR ANY OTHER WRITTEN NOTICE, OR SEND A TELEGRAM TO IJL ST 1800 WEST LOOP SOUTH, SUITE 490 HOUSTON, TX 77027 NOT LATER THAN MIDNIGHT OF_______ (the third business day after the date listed above).

Relocation Policy per the Contract:

If Client relocates his/her primary residence further than 50 miles from this IJL office and is unable to transfer the contract to a comparable IJL office, Client may elect to be relieved of the obligation  to make payment for services other than those received prior to that relocation, and if Client has prepaid any amount for dating services, so much of the amount prepaid that is allocable to services that Client has not received shall be promptly  refunded to Client. If Client elects to be relieved of further obligations pursuant to this paragraph, Client shall be charged a relocation cancellation fee of one hundred dollars ($100) in order to establish eligibility for relocation benefits under this section, the Client must present a valid driver's license from their new residence.
