A complete response to this collection letter is attached (including numerous documents). In short, I had several surgical procedures done at the *********************** LLC in north ******* in 2021 and 2022 (dates were 12/13/2021, 12/27/2021, 02/14/2022, 02/28/2022, 03/21/2022 and 03/28/2022). These procedures were performed by *************************************************. ************** elected to perform these procedures at the *********************** LLC located at **************************************************************. My primary insurance was ******** and my secondary insurance was ********** Blue Shield Federal Employee Program (FEP). ********** Blue Shield coordinates coverage with ********. ********** covers the charges (including deductibles and co-pays) that ******** does not cover. In the case of these six surgical procedures, ********** paid a total of $691.54 (as detailed in the attached documents). Because the **************** Center, LLC was "out-of-network" for **********, they sent the checks directly to me so that I could pay *********************** LLC. My husband personally went to the *********************** LLC and handed them personal checks for these charges on two separate occasions. On 04/04/2022 we gave them personal check number 7379/1221 in the amount of $$126.60. That check cleared our checking account on 04/07/2022. Then on 08/02/2022 my husband hand delivered a second personal check to *********************** LLC. That was check number **** in the amount of $564.94. That check cleared our checking account on 08/09/2022. A copy of that check from the ***** Fargo Banking on-line web site clearly shows that this check was deposited by *********************** LLC into their *************** account. The total that we paid *********************** LLC was $691.54 ($126.60 + $564.94 = $691.54). Now, a year later, the Patriot Compliance & Recovery Services, *** (a collection agency) claims that we never paid this $691.54 and is threatening us with collection actions. Please note that we have never received a bill, statement, letter, email or phone call from the *********************** LLC or the Patriot Compliance & Recovery Services, Inc. It is odd that a year after we paid these charges in full, we receive a collection letter attempting to collect that same amount a second time. Please review and let us know if you can assist us in resolving this matter. Attachments.

I was sent two medical bills by Patriot Compliance that were from 2021 - three years old . They were a list of co-pays they claimed were unpaid to Oconee Regenerative &Joint for a total of for . I contacted them at a man named ************************* answered and I told him that I had proof of all of those charges having been paid on those exact dates in2021. My credit card sends me a summary every year of all charges made categorized .and under medical all those co-pays were paid on the exact dates they had sent on their bills plus and I inquired were they aware that the *************************** had disappeared and were no longer in existence ? My husband and I returned there and they were gone ! No one was there anymore ! When my husband and I had gone for knee injections we paid all co-pays right up front !it was required ! The girl at the front desk always took your credit card and charged the co-pays before we ever went in!!! Well *********** states that h*** investigate and then that evening proceeds to simply send me an EOB from that year which everyone knows is no proof of payments and tries to state that that was their proof ! I promptly blocked him when I tried to email him my credit card statement showing all those charges had been paid and then the email www.patriotcompli.com indicated that it was not a working email when I tried to send it ! This ,I believe , was an attempt at fraud regardless of the credit rating the BBB has attempted to give this company ! Please let me know what you know about this situation !
