            I requested my medical records and i had to pay $54,75 because i couldn't get any information on my *** *********************************** They told me she abandon them and they have no information on how to reach or where she is. i was told it was .75 cent a sheet and i ask why didn't she copy double sided said Dr in-charge said do it that way. about 20 pages doesn't have date of service on them only date they printed them. i need medical clearance for surgery and i would like to be contacted by ******************* or given information on how to reach her as soon as possible. i have been a patient for over 20 yrs. i would like to be refunded my monies because if doubled it would have been half that price.             My blood pressure medication prescription expired, and was told I have to see doctor before they renew it. This is usually done on phone. Instead I was force to use vacation time to go to office, charged a $20. copay. The doctor said that my prescription should have been renewed, since I have a pending physical in 1 month.The receptionist (in a very unprofessional manner) told me go to urgent care to renew my prescription.
