LTC Services can be provided in your own home and community, depending on the level of care you need. Faith and Hope Independent Living, LLC offers alternatives to long term nursing home care by providing supplemental supports and services, to eligible individuals in the home and community-based setting. Individuals must meet specific age, disability, and nursing facility level of care criteria/requirements, as well as meet Medicaid financial eligibility requirements.
