    1.Date of the transaction:July 6, 2023 2.The amount of money I paid the business.$16,700.00 3.What the business committee to provide you.Removal of two front teeth located in the front right side of the mouth and one(1) broken off tooth located in the upper back left side. Replace bottom back teeth with implants as well the broken off tooth located on left side of my mouth, replace two (2) front teeth and replace them with implants.4.What the nature of the dispute is. I was told that I was going to walking out with implants the same day. We are now in the month of December 2023, and I not yet receive my implants. I was told that they wanted to wait for my mouth to heal, which is and was a lie because I was supposed to have received my implants the same day.5.Whether or not the business has tired to resolve the problem.The only thing that the business is trying to do is to get me to pay another $8000.00 dollars. They have refused to allow me to get into my account to see what is going what is the problem. I was told by the owner ***********************, that she was going to reach back out to my insurance company and that was back in October, and I have not heard back from her to this day.6.Account/order/tracking number.Account is *****************************, with no order/tracking number.7.If the issue involves advertising when and where the ad was seen or heard.I have on recall of any advertising. I found her online.

    ************** signed forms (see attachments) that stated she understood that if her insurance does not pay that she is responsible for the remaining amount. The forms also stated that the treatment plan she signed is an estimate of what she owes, not a guarantee of all she has to pay. She also signed a consent form that states she understood she would receive extractions and implants and if teeth were not able to be attached to the implants that day she would be provided with a denture. ************** was unable to stay still towards the end of her treatment despite numerous attempts to help keep her calm. Her implants were placed (see attached x-rays) even though it was difficult to get her to cooperate. Then the lab technician who was the in office to fit her teeth was not able to complete his work due the patients moving too much and hitting the technicians hands. We completed as much work as was possible that day. ************** was given her denture as temporary teeth because the implants needed to be in the bone 3-4 months before we are able to place the fixed teeth since she was unable to cooperate with getting the teeth placed the day of surgery. We billed ************** insurance and filed multiple appeals,but they denied her treatment. She currently owes us about $11,000 due to the insurance not paying which she has refused to pay. I am willing to complete her treatment once her balance is paid, or even attempted to be paid with a payment plan but she refused. We reached out to her about financing options and ways to help her pay her bill. We are trying to work with her even though she has yelled and cursed at my staff. I am still willing to finish the work and just part-way after that as it has been difficult working with her. Unfortunately, since her insurance did not pay, she is responsible for the remaining balance as was stated in the forms she signed.

    I would like to express my gratitude for you checking into this for me. I will not be switching between Premium Periodontics Dental Implants because I paid for a product that I have not received and will not be getting any time soon. The process has not been completed as agreed upon by Premium Periodontics Implant. In the fall of 2022, I contacted Premium Periodontics to set up a meeting with ***************************. When I visited her, I I made sure to let her know exactly what I wanted.
    I told ************** during our consultation, I wanted to get implants to replace the teeth on my upper and lower jaws. We decided that she will examine my entire mouth during this session in order to determine what I required and desired. ************* recommended that I have my two front teeth extracted following this examination because they had discolored teeth. I agreed with her that I would be willing to have it done and that I also needed her to extract a fractured tooth that had broken during an earlier root canal.
    I went over with **************, that I wanted broken, tooth replaced, which was located on the upper top left of my mouth.
    There are two back teeth or molars, on each side and a total of four (4) bottom teeth or molars, on each side of my mouth.
    What I got were dentures for the upper mouth and nothing for the bottom mouth. She did not do what I came in for.
    My mouth was so sore and swollen that I did not know what to do with the dentures she gave to me or when to use them.
    I went back for my check-up to show how I was healing, and they said that I was healing properly.  They never mentioned to me about how my dentures were working out.  And I will admit that I never mentioned anything about the dentures because I knew that I only had another month before I was to come back and get my implants, at least that what I thought when they told me that I had to wait for three (3) months which I still did not understand.
    I was under the understanding that I would be getting my implants on the same day.  This is why all of this is a surprise to me because I had no clue.
    I called the office to ask what the next steps were and to find out about what to do with these dentures, but they overrode me and just kept talking about how much I owed.  I got so frustrated because I, needed answers because I had no teeth at the top of my mouth, and I did not know what to do with these dentures.  They would not let me finish asking questions they just kept talking over me about what I had to pay, and they were not going to help me with anything else unless I paid the balance.  I was deflated I just hung up.
    I feel like I have been ripped off and yet what I asked for has still not been completed.  I have to walk around with a mask on every day because I have no teeth.  This is unacceptable.
    This has greatly affected my life. I can only see my entire face when I am at home.  I do not even like to be in public much.  My self-confidence has been affected and I need help!!!
    After I have tried the dentures for about a week, they were just too bulky at the top and it made me talk with difficulty. I was unable to be understood or for anyone to understand me unless I removed the dentures.
    If you do not help me, I have no idea what I can do.
