    I found the practice online in need to get a regular check up examination. I called the office a day before my appointment on 02/16/2022 to reschedule my initial appointment. They advised me on 02/15/2023 to come in that day for my appt. After reviewing i realized I couldn't make that appointment. I reached back out to the office to reschedule it, receptionist on the call advised me that they were going to initiate a $50.00 cancellation fee. I did not agree with that being so the appointment was just made moment ago. They waive the fee and told me come in tomorrow. When arriving at the office at 12:27 to fill out new patient paperwork. the receptionist told me to wait and I would be called back momentarily, no paper work was giving or. I was not asked to sign any financial agreement, HIPPA, medical history or any information pertaining to my health which should be asked at any appointment by a practice per law. When going to the back i asked her if i needed to give me information she told me i was a recurring patient. I advised her I was not. When i was handed the forms she asked me for pharmacy information . I told her the name and road and city of my pharmacy she then argued with me stating she's not from here and she wouldn't know my pharmacy and I need to find the full address and provide it. another employee interrupted and asked me to provide a urine sample. After doing so when leaving the restroom the office staff was congregated in the hallway discussing something. Then multiple employees called me to the front and told me again i needed to get pharmacy info as i was doing so the doctor informed me she would not like me as a patient and its not her job to look up my insurance information. I asked for my papers to be shredded and left. This experience was unprofessional and very concerning being so this person is dealing with someone health and not even gathering there information prior to seeing them.

    Dispute this amount $350. This is a wrong amount. They are sending me collection notice and threatening me about collection. Keep sending me medical bills which i was not aware about it and also I do not believe this amount is due. They just made a bill and harassing me. I am an auto accident patient and also i provided my both health insurance details to them. Still are disturbing me. I cant pay this amount and also i cannot take this harassment
