    The company was an ** clinic and asked me to sign a contract for $6,000 before beginning treatment. I went to the *********************** location until early 2024, when they closed their doors and had no forwarding address nor phone number. I had been paying monthly but just had to pay my balance to Primis, the bank who loaned the money to me through Maple Grove.

    Maple Grove Medical Clinic opened a loan in my name without permission for services not rendered. I went to the clinic for a free consultation. I called the next day to tell them I was not interested. That was in October 2022. I JUST received documentation that they have processed a loan in December 2024 Two years later! For services never provided! The loan documents say I owe $10,869.18. Totally untrue. I these people are crooks and are taking advantage of hard working people. Its disgusting!

    On February 26,2023 I went for a free consultation to a Maple Grove Medical Clinic at 5 Park Court in Ownings Mills, Maryland 21117. There were no serves provided after the initial free consultation and I have been notified that a loan from ***** **** ******* of $6,500.00 has been opened in my name for services at Maple Grove Clinic. Maple Grove Medical Clinic provided absolutely no service(s) to me and there were no additional appointments ever scheduled beyond the initial free consultation. I signed nothing to open a loan for Maple

    Wave therapy treatment Zero results from treatment
    Many times I went to the office in times 2022 the office was closed without notice in June of 2023 closed without notice (. I found out though your site that they were out of business)
    they financed there treatments though a company out of Georgia called ***** Loan # ********** phone# ************** e-mail *******@*****.com. Maple Grove advertise on ****** ******** and was interviewed by Host **** *. Also local radio stations carried there advertisements so i thought it was legal company. ***** expects payment by 09/20 or it goes up to $ 6000.00 >

    My husband went to Maple Grove Medical Clinic for ED treatment on July 23, 2022. He received his initial consultation and then they offered him his first treatment. (Note: They originally scheduled an appt. for his 1st treatment in August, 2022.) He accepted, but later that night, he decided he did not want anymore. Fearful, uncomfortable and anxious, he informed them of his aforementioned decision on the 2nd day. They initially seemed to work on voiding the contract, and stopped the upcoming Autopays (ACH) from the checking account. He spoke with Representatives on at least 3 occasions regarding the same; all of them stated that a ticket was generated for the verbal (call) transactions. Subsequently, he received a call from Rep. Simone with Universal Account Services who stated he could not cancel the amount owed ($3,500) because he accepted one (1) treatment. My husband informed them of the above and that he was not pleased with the treatment. He even stated if he had to pay for the 1 treatment, he would pay that amount. (Note: My husband has paid NO monies to date.)
    He spoke with Dr. Andrew ******** re: confirming cancellation. To this day, my husband has NOT had any future treatments, but still receives billing notices with late fees attached.
    Your help in this matter is greatly appreciated in cancelling this UNFAIR contract and having NO FURTHER communications with Maple Grove.

    I signed up for ED services. I wasn't not seen by a professional doctor. I was promised 99% success rate. Then was told to wait for the financial assistant to come in. I was met by a very unprofessional person. I agreed. $6000.00 in a payment plan. You have to make appointments to be seen. On numerous occasions I waited over an hour and a half, when I expressed my concern, the answer," these clients came early, you will be next". The persons that do the procedure aren't medically trained no doctor on-site. No consistency in the treatments. Expressed my issues with Jerimiah "manager" said he would preform treatments personally. Did not. I was told email for a resolution. Supposedly in fine print, never discussed , once signed an treatments start no refund. I responded the biggest scam. I informed going to the local news they advertise on. Response, "ok, thank you " Very professional. I've asked for a contact and number to the owner/manager with no response.

    Maple Grove Medical Clinic advertises on television and radio for Acoustic Wave Therapy to correct Erectile Dysfunction. Our advertised offer includes a blood flow ultrasound to determine the severity of a patient's situation and an examination from a medical professional to ensure someone is fit for treatment. All of this is performanced free of charge. After hearing our advertisement Mr. ********* contacted our scheduling to make an appointment to learn more about Acoustic Wave Therapy.


    On July 13th Mr. ********* came into our place of business for his consultation and all of the free services Maple Grove  provides we performed. After our clinical staff presented our offer which included the specific number of treatments recommended by our medical professional and the exact dollar amount it would take to perform them. All of this information is verbally given as well as a physical document as well. 


    Maple Grove does not require a same day sign or and there is no obligation to engage in the service during a patient's consultation. At any time Mr. ********* has the opportunity to review and decline Valley Park's offer. Instead Mr. ********* chose to move forward with the treatment. Mr. ********* took advantage of one of our 3rd party finance companies for assistance and acquired a loan to pay for the treatments. Mr. ********* willingly provided us with his full name, phone number, address, DOB, and SSN. Maple Grove  executed the transaction in good faith and with Mr. *********'s consent.


    After the transaction was approved, Maple Grove  offered to start Mr. *********'s treatment plan that same day. Mr. ********* accepted and was escorted to a treatment room where one of our technicians performed his first treatment with him. Mr. ********* then came in several more times on different dates to receive treatment. Maple Grove  offers a medical procedure that unfortunately does not have a 100% rate. Maple Grove  does not guarantee results or make promises of outcome of any kind. Although Acoustic Wave Therapy has a high success rat there are a small percentage of patients where the process of neovascularization happens in minimal amounts that it does not produce noticeable change. 


    We are always happy to continue working with Mr. ********* in his pursuit to correct ED. We welcome him to come back to our place of business to receive some additional treatments at no cost.

    I am rejecting this response because:

    I agree that I did willing make an appointment.  Once there, I was escorted into a room, nothing clinical at all. An Asian male entered the room to do an ultrasound. He wasn't a doctor nor did really introducing himself.  He stated that he was to perform an Ultrasound.  Not explaining what he was looking for or explaining what a poor or good diagnosis is.  I had to take his word for it. He proceeded to tell me that I'm a good candidate for the Wave therapy.

    He left the room.  A poorly maintained older man then enters the room to discuss financing. Again, not a technician or doctor and smelt like an ash tray. With his broken New York accent tried to bring up conversation due to my last name being *********.  Yes, I agreed to move forward due to the Asian man stating I was a good candidate for this. Anyone with my condition is embarrassed any looking for hope. I was at a vulnerable state of mind, which they feed on.

    I was then escorted into a non-clinical dirty room with broken "call it balloons" on the floor and table. These are like small prophylactics to go over end of the device they use. I was asked to get undressed and get onto a table and put my feet in the stirrups like giving birth women do.  I believe his name was Jermiah (office manager) not a doctor or medical technician entered the room and changed the prophylactic on the device.  With an already opened and dirty bottle of lube that has been used on others on their private areas, proceeded to apply it to me.  We did my first treatment. My thoughts, ok that is what's to be expected.

    I made another appointment. I get there 15 mins in advance as asked. I waited for about a half hour after my appointment time to finally get called in. A younger male escorted me to the same dirty room. He is to perform my next treatment. Asked me to undress and assume the position. Ok, he then leaves the room to go get the cart with the broken balloons and dirty bottles of lube previously used on someone else, opens the door fully to enter while I'm naked for anyone in the hallway to see me "embarrassing" and proceeds to spread the lube on my private areas and started the treatment. Compared to my first treatment, there wasn't any consistency of placement of the device and the device at times was just laid on me vs being pointed at my body. And in between placement the device is still in operating status, meaning I was missing the sound wave impulses.  You are to receive so many in a given spot per Jermiah.

    After the treatment, I went to the main desk to schedule my next appointment.  Jermiah was working the front desk and I explained I wasn't happy with the way this treatment went and why. He assured me that he would give me a free treatment he would personally do the treatments moving forward himself. So, I made another appointment.

    I show up on time once again and wait roughly an hour this time past my original scheduled visit. They didn't have "technicians" there yet.  Finally, I get called in.  An older heavy set guy escorts me to the same dirty room as asks me to undress and assume the position. He leaves the room to get the same dirty cart and opens the door as previously stated to awkwardly getting the cart in room, and he then proceeds to do the treatment, not Jermiah.  To say the least, this was another inconsistent treatment compared to the first two. I expressed my concerns once again with Jermiah.  He apologized for incident. And assured me that he would take care of it.

    Several more treatments in, the inconsistencies continued, and nothing changed.  The last straw was upon my last appointment, I arrived 15 minutes ahead of time as asked, turned off my cell phone as requested per the sign.  There are several older gentlemen in the waiting room talking on their cell phones. An hour and a half later past my appointment time.  I went to the front desk, and inquired about why am I waiting so long. The unprofessional response given to me was that these persons showed up early to their appointment times we are seeing them first.

    The lack of no medical technicians or official doctors on staff, the lack of hygiene, this place is a scam on the vulnerable.  I asked for a contact number to speak with a true manager or doctor.   I was given an email address to attend my disgrace. And I walked out of the office with no treatment.  I emailed my concerns. And this is the unprofessional response I received.  Its attached.

    -I saw a T V ad for ED, made an appointment and had my initial 1st visit on Sept. 8th 2022. at 1:pm in which their Salesman, Chris, met with me to enroll me. I told him I was on a fixed income and did not have much money. I tried to get into their system but was unsuccessful so he took the phone from me and checked my credit then tried to sign me up. He tried 3 times and was unsuccessful until the 4th time he got an approval to which he was very excited, He told me it would only cost me $150 per month (never giving me what the total sum would be). While he kept trying to get me in I told him I had an acupuncture appointment at 3:pm but he ignored me and kept working my phone. At about 4:pm he had me sign into my phone which at that point I was making a contract. He then had me wait so a tech could do my 1st treatment.

    I felt that I was obligated to move forward since he had me stay for the 1st treatment. I called and canceled the remaining treatments on October 13.2022 and called and talked to a man who said he would pass on my concern and have someone get back to me within a week. On October 24th I received an email from Jon, a supervisor asking me to give him an email back with my issue. I emailed him back that I wanted to speak with him personally to give accurate info, On October 27th @ 5:44 he called me back. We talked and he said I had signed a contract and was obligated to pay the $3538.34, I told him I was on a fixed income with the minimum SS Benefit. and a small part time job In the end he said he would lower the pay out by half which was $1750 .00. I told him I could not afford that.

    We left it at that and I said I would be in touch with the Maryland Better Business Bureau and see what can be done. I received my 1st statement about October 30th and saw a very high interest rate on this bill. So Chris in order to get me signed up kept going till he found a lender who would take me but for a higher interest rate without my knowledge. That is what I call a predatory salesman who's sole purpose was to make a sale by any means possible and without giving full disclosure to me. Chris also told me I could call him any time if I had a problem. I had called him 4 times when I was troubled by this deal and he NEVER returned my calls. He made a sale and that was his sole job by any means possible to get his commission. So how much is that total bill I am supposed to pay is his commission?

    I know it has taken me a while to get around to this complaint but life has been difficult these last few months and just am taking the time to put this together has been a difficult task as I am 75 years old.

Mr. **** decided to take advantage of our offer and schedule an appointment on September 8th



    After the services in our free consultation are performed, our clinical staff presents our offer which includes the specific number of treatments recommended by our medical provider to work on correcting ED, along with an exact dollar amount it would take to perform them. With the offer, is multiple forms of paperwork including our refund policy, consent to treat, and patient release and liability waiver. All of this information is presented to each patient and Mr. **** was no exception.



    All of the documentation listed above was given to Mr. **** for his review. At any time Mr. **** has the ability to review and decline our offer. Instead Mr. **** decided to move forward with our procedure, sign all paperwork provided and give us his full name, phone number, DOB, SSN to have the procedure financed. The transaction was done in good faith and with Mr. ****** consent.



    Mr. **** made his complaint and request known to our patient relations department in hopes of cancelling his financing and relieving himself of the financing responsibility he has taken on. Patient Relations spoke with Mr. **** through email and by phone to discuss his issues and review the terms and conditions Maple Grove Medical Clinic has included with their service.



    Mr. **** explained his financial hardship he has been experiencing with our patient relations department and Maple Grove offered a compromise for reimbursement for a disclosed amount. Allowing Mr. **** to be relieved of some of the financial responsibility he had initially agreed to. We are still open to working with Mr. **** to correct ED and are committed to finding a compromise to help him.

    I went to Grove Medical Clinic for treatment for erectile dysfunction I sign up for 4 treatments at a cost of 5400.00 dollars after my first two treatments I told the staff I saw no change sexually when I went for my third treatment, I explained this to the staff member that was going to do the procedure his response that I may need to sign up for treatments 7 more treatments that would have cost 7500.00. The treatment is supposed to be a wave treatment after my third treatment something went wrong I don't know if the tech turned up wave volume but I have been in a lot of this discomfort I called their office the next day to let them know how I was feeling and was told to give it a couple of days sometimes discomfort happens well it has been a month and I am still felling the same I have called their office and spoke with the nurse and the thing she could tell me is there studies show not that something couldn't happen I think these people are frauds and should be investigated
