    Took my daughter for birth control implant "*********". She had consult on 4/8. At that time, the physician and I decided on the ********* implant, I filled out the request to give my consent and order the implant and was told the office would contact me after the pharmacy ordered and when it arrived. After 1 month, I called/left messages for office manager every week without calls being returned on a status. I finally spoke to receptionist the week of 6/17 and was notified the implant was available in their office. An appointment was set up for 6/24. On 6/24, my daughter and I arrived, saw the PA and she then explained the proceedure was a 3-month implant and it was determined they had the incorrect birth control, it was to be a 3-year implant. I refused to go back and requested refunds of my office visit charge for services not rendered, which they refused. I lost over 10 hours lost wages ($500) and co-pay ($40) for services not rendered. I had to locate new GYN, putting this procedure off another my daughter is suffering severe menstrual cramps an additional 2-months (which ********* is used to control) and will be losing another 10-hours of work. Requesting lost wages and office visits co-pay. I should be requesting payment for my daughter's pain and suffering. This health care provider is there to provided medical care which was not rendered.

    On said date of 4/8/2024 patient came to office with her mother for an appointment. The patient requested a Nexplanon implant. On 6/13/2024 the patient scheduled an appointment for 6/24/2024 for the Nexplanon implant, however, the appointment was scheduled for a 3 month Depo Provera injection. The mother stated she filled out the Nexplanon order form at the 4/8/2024 appointment. However the form was never received by the representative who places the order for said device. I, spoke with the mother and informed I could order device and she stated "no I will never come back her, I will take her some place else".

    After Explanation Of Benefits received, it was deemed no copays for that date of service was owed, $40 check issued to patient's mother and was cashed on 7/18/2024. No charges was billed for date of service 6/24/2024. This office will not reimburse the mother for lost wages for bringing her daughter to a scheduled 2:35pm office appointment.
