To Whom it may concern. I was shocked to discover a collection account from Avante ********************* on my credit report via Credit Karma with no prior notice or communication. Upon investigation, I traced the origin to Inphynet ***********, allegedly affiliated with ********************* of Broward, who denied any record of me. I was then referred to TeamHealth, who also confirmed no records exist.This points to a likely pattern of fraudulent collection practices. The BBB shows over 50 unresolved complaints against Inphynet ***********, many similar to mine. This conduct appears to violate the Fair Debt Collection Practices Act (FDCPA) and raises serious legal and ethical concerns.I demand:Immediate removal of the account from all credit bureaus.A full investigation into this erroneous report.Written confirmation of resolution.Please advise if I am within my rights to file a police report and pursue legal action for fraudulent reporting and defamation. On or about 10 15 July ********************* to the ***** General ************************* *********** to get medical attention for severe leg pain caused by taking Lipitor. During the visit I provided copies of my ******** card and my retired military ID showing as proof that TRICARE For Life was my ******** Supplement. around 27 October 2023 i received a bill from TeamHealth ******************** asking i pay them $1524.00 I noticed that the bill stated that TRICARE for Life paid $0.00 and there was no mention that TeamHealth has billed ******** first and then TRICARE for Life as required, since they are my supplemental insurance. The same day i contacted TeamHealth and asked if they have billed ******** First, as i am over 65 and ******** is my primary insurance? the person i spoke with told me they declined the payment. I told the person, that according to TeamHealth statement, only TRICARE For Life was billed and that there was no mention of billing ******** for this service. I proceeded to inform the person that since ******** was my ****************** they have to bill them first, and then bill TRICARE for Life for the ****** that ******** did not cover. I also told him that once TRICARE for Life paid their allowable ******, I will pay the max ****** stipulated by ******** after both ******** and TRICARE for Life have paid their allowable. I also asked the person on the phone to send me an itemized bill for this service, as one was not included in their letter. The person put me on hold for a while an can back and say that i owed that owe them $1524.00 because the payment was declined, and again i told him that If they have billed ******** first as required, I would be able to see that transaction in my ******** account or at least receive a notification from them as to what ****** was paid or why it was not paid. Today i received a letter from HRRG asking for $1524.00 payment, but nothing about billing ******** or Tricare.Please HelpInitial Complaint
