            I went to the ER at Mission Viejo hospital in June 2020. I was treated and paid my copay. In November 2021, I received a letter from **** saying that I was sent to their collection agency by Mission Viejo Emergency Medical Associates. I never received a bill from them. I immediately contacted **** and Mission Viejo Emergency Medical Associates billing department. Since November 2021, my insurance company ****** and I have been trying to resolve the issue with Mission Viejo Emergency Medical Associates but the billing department has been very difficult to work with. My insurance is asking for my medical record to process the claim but the provider has refused to send it again. I have called them 5 times since November and this issue has not been resolved. Anthem and I would like the provider to get paid but they have not been cooperating.
