On 8/10, I was charged $75 by SimonMed. After receiving notification from them charging me additional money, they realized that they had misapplied my payment and I should have initially only been charged $50. SimonMed acknowledged they owe me $25 and have yet to refund me. I have called numerous times and each time I am assured that the refund is being processed and it should take **** business days. It is now 4 months later and I have still not received the refund.Initial Complaint

On 1/19/2023, ***** Med preformed a Screening Mammogram/Read that was to be 100% covered by my Insurance. Apparently, the insurance denied their claim for reimbursement because Simon Med did not submit the claim to my insurance company till late September. I discovered this when I started receiving text/emails for $971 amount due on my phone late November. There was no explanation of reason for bill, and only after calling Simon Med and my insurance did I discover that Simon Med had 15 days to bill my insurance per the obligation agreement, and they failed to do in time allowed. I have disputed this with insurance just last week, and I was told by Simon Med that they would put this bill in a 45 day freeze while this bill would be resubmitted to insurance. However, I have since recieved weekly reminders my bill is due by the end of the month (Dec 2023). This needs resolved immediately, and is causing a great deal of stress.Business Response

This is against Simon Medical for an MRI they mistakenly submitted it wrong to the insurance carrier and we had to pay on the spot and was told it would be submitted and reimbursed, they now say since we paid cash they can't submit it. They admitted they made the billing mistake and now won't submit the bill so I can be reimbursed. I've attempted to *** them in small claims court but they don't provide a statutory agent so it becomes a wild goose ****. They've wronged us, this was for my wife fighting stage 4 cancer and $475 is a lot of money to us to have to not get it reimbursed on top of all the other huge medical bills we pay. I spent 2 hours on the phone today with BCBS Insurance and Simon Medical, they have never once attempted to contact me, I have called them dozens of times in the last 2.5 months to resolve this and they have refused to help, the one time I was able to reach them by phone, they said they would let me know and I've never heard anything. Only today did they respond to BCBS and after a 20 minute call with them trying to resolve this matter, they hung up on us and then another 20 minutes later we reached them again. They are being beyond difficult and not honoring the contract they have with BCBS and only after fighting with them for over an hour did they agree to submit the claim in order for us to be reimbursed. This was their mistake by their own admission and they refuse to cooperate with us. After reviewing them on line, I see hundreds of similar claims against them. I would think the BBB would be more interested in protecting the people for their sleezy business practices.Business Response

simonmed is double billing.my insurance CIGNA said they paid everything owed. I paid 530 and they still want more. Someone needs to check in to simonmeds billing methods. don't tell people their insurance allowed the procedure when they haven't yet. Get clearance then do the procedure. On December 1, 2022 I paid SimonMed a copay to receive my mammogram imaging. My insurance has since paid them and has informed me that SimonMed over-charged me for my copay. I have call several times, the last time was on 11/01/2023 Reference #TONI102023. On 11/01/2023, I was finally able to talk to a manager, who said she would get me a refund, but like in all previous calls, my refund has never come. According to my insurance, ******************* SimonMed owes me a refund of $95.25. I have called, emailed and sent copies of them copies of the Explanation of Benefits for these claims and still they promise me refunds, but I never receive my money.Business Response

On Aug 15,2023, I had to get an MRI and paid $1,440.72 because of not being able to get authorization. My health insurance, Aetna, explanation of benefits came in , stating the cost should be$641.25. Simon med stated they would refund me in 45 days, not including weekends. On Nov 21,2023, I called Simon Med that my $799.47 refund had not been deposited. ******** said she will expedite it in **** days. Hello I have attached a bill from Simon Med for a service provided on 01/03/23. Simon Med billed it to the wrong insurance not sure how or why this happened, however, I had to pay the $670 as I kept getting harassed and threatened that I would be send to collections. After working with my insurance and Simon Med admitting their mistake, they informed me they would re-submit the explanation of benefits to my insurance. This has been going on since July 2023 and I paid the full amount on 07/19/2023. I have kept a thorough record of everyone I have spoken to with dates and times. I kept being told I would get refunded after paperwork has been submitted and they have yet to do that. I spoke to my insurance, and they informed me that the paperwork process has nothing to do with me and they should refund me and if I owed a co-pay I would be informed. My insurance has also been speaking to Simon Med staff and they give them the same excuse they do to me, no resolution whatsoever. I need to be refunded and I am also in the process of filing a lawsuit with my legal team if this does not get resolved immediately.Initial Complaint

SimonMed Imaging, ***** ***************************., #***, Phoenix, ** ***** SimonMed Imaging was recommended by my doctor. I called SimonMed and was scheduled for a CT scan of my spine on 6/28 at the above location. Prior to my appointment, I went on-line and completed all the paperwork, including insurance information.6/28/2023 Upon checking in at SimonMed, the receptionist told me my co-pay was $150. She said it would be refunded to me in 2-3 weeks if incorrect. As I waited in the reception area, I heard the receptionist demanding copays for as much as $500. When I got home from my SimonMed, I called ******************* and spoke to CSR ****. I asked him if I owed a co-pay. He replied, Were you dealing with SimonMed Imaging? **** confirmed that I did NOT have a co-pay. 7/30/2023 I received a text and an email from SimonMed Imaging telling me I owed them $1,393.49 and it had to be paid on-line by 9/8/2023. 8/1/2023 I called ****************** and spoke to CSR, ***, who said that *** sent a check to SimonMed on 7/27/2023, for the allowable amount and I should receive a refund for the $150 deductible I paid on 6/28. CSR *** called SimonMed Imaging with me on the line. He confirmed with SimonMed that they received the *** payment. I was told a refund of $150 would be processed and refunded to me.9/6/2023 I called SimonMed Imaging and spoke to ****. He said your refund is in process and that I should receive a credit on my credit card within ***** business days. Timeline / Conclusion:I paid an incorrect $150 deductible on 6/28/2023.****************** sent the total allowable amount to SimonMed on 7/27/2023.On 8/1/2023, SimonMed confirmed they had received the payment from ******************.Per my 9/6/2023 conversation with **** at SimonMed, the refund should be received on my credit card within ***** business days. 30 more business days from September 6 would be October 18 45 more business days from September 6 would be November 8Customer Answer

On 1/6/23 I had 3 MRIs performed. One insurance covered the other two were out-of-pocket at a cost of $475. The insurance company was billed for the brain MRI, but instead of utilizing the funds that the insurance had paid for that MRI, Simon Med opted to take my out-of-pocket payment made for, the two additional MRIs and apply it to what they stated was owed from insurance. When informed that they were not authorized to apply an out pocket fee to what insurance had already paid (double dipping) I was told they would correct it. My deductible is $600 and premiums is $600 both were met in May 2023 yet Simon Med charges me $193 (5/20) $49.60 (5/22) and $171.39 (11/13) totaling an additional $460 over my deductible amount. When referencing my explanation of benefits and asking for itemized bills, I received 3 different statements with inconsistency in the information provided. When speaking to representatives, I was told the billing department would address this and make the correction. My insurance will not reimburse my $475 out of pocket payment without accurate statements and reimbursement from Simon Med cant occur when they charged for deductibles that they were not entitled to receive. I have spent 11 months attempting to remedy this billing issue but cant get one individual who works in the billing department, and any person who attempts to address the issue has no authority to address it or provide a true explanation. Ive had two individuals from my insurance company on 3 way calls with Simon Med only to still sit waiting for a resolution. Making up fees and not being able to keep track is evident by the multiple invoices Ive provided. Charging my credit card without adding what the charges are for is how Simon Med continues to evade responsibility for their billing practices. The manipulating when payments are disbursed would constitute fraudulent billing practices. Delaying refunds for months on end also indicate nefarious billing practices.Business Response

The date of service rendered was 08-31-2023 its for a dating ultrasound. I called them a week before to schedule an appointment, I was told I have to wait a week as they are going to send my insurance authorization for this service. 08-31-2023 My husband and I arrived at Simon Med in ****************. They charged me $143.08, I was confused because I know I shouldnt have any copay. I called Anthem BCBS and they told me Simon Med didnt file the claim, I called SMI for them to file the claim and for me to get refunded. SMI filed the claim and was approved and paid by my insurance, when I called to ask for my refund they told me to wait 45 days that was September. It is now November 27th and I still havent gotten my refund. I called again today and spoke to ***** F customer service rep I was given a refund reference/ batch number ****** and I was told to wait another **** days. She told me that my refund was processed 09-22-2023 and should be received electronically as I used my debit card to pay, I asked for a supervisor and ***** spoke to me, she explained that it was processed 09-22-2023 but fell off their system because nobody approved it within 30 days, she was giving me the check number but I told her I was told that it should be done electronically and not by check, the ** Rep and the Supervisor was not even on the same page. Basically SMI was doubled paid by me and my insurance but wont refund my money. I have already waited more than 45 days and its almost **** and still no refund, my ultrasound was done end of August, I shouldve not get charged if only they file the claim like they told me they will in the first place but did not. I would like to get my refund ASAP or else Ill have to get this escalated and file a lawsuit.Business Response
