The following describes a pending government action that has been formally brought by a government agency but has not yet been resolved. We are providing a summary of the governments allegations, which have not yet been proven.
On January 3, 2020, the Arizona Attorney General filed an Indictment against Michael L Ham, owner of Kokopelli Eye Institute. Charges filed allege Dr. Ham is in violation of State law prohibiting conspiracy, fraudulent schemes and artifices, illegal control of an Enterprise, theft and money laundering. Ham is accused of fraudulently billing Medicare, the United States Department of Veterans’ Affairs, and the Arizona Health Care Cost Containment System, aw well as several private health insurers, for hundreds of thousands of dollars between the years of 2009 and 2018.
The matter is pending.
