              On 6/28/22 I visited ** Dermatology for annual visit. As an output they recommended a prescription for hair loss, however, the prescription may not be issued without blood work confirming healthy prostate. Due to side effect I chose not to pursue blood work nor the prescription.On 7/16/22 ** Dermatology arbitrarily issued the order for the prescription to CVS. It was bundled into our other prescription pickup and was not noticed until returning home. Immediately call ** Dermatology who acknowledge the oversight and told me to immediately discard the pills, however, said they would not issue reimbursement for the prescription.Location: ***************, ** branch **********************************************
