Under Internal Revenue Code sections 3402(f)(2) and 6109, you are required to provide this information so we can determine the correct amount of federal income tax to withhold. If a properly completed form is not provided, you will be taxed as a single filer with no withholding allowances. Providing false information may result in penalties. The information on this form may also be shared with the Department of Justice for litigation purposes, state and local tax authorities, and the Department of Health and Human Services for the National Directory of New Hires. As a reminder, your base pay will be $4200 per month + $35 in bonuses (based on performance, assuming all packages are processed on time), before taxes.
