      I have a workmans comp claim dating back to May 2019. My attorney and I have been trying to get vital information from Theda care and related Doctors. It took 1 1/2 yrs to get the paperwork after a multitude of excuses from Theda Care. We are now down to 1 last piece before officially filing a workmans comp claim. On 11/28/22 i made a special appointment to talk to the doctor directly about the last form. it had 3 boxes for him to check and sign his name. He told me that he would give it to secretary and 4-7 days I could pick it up. I called in 10 days. No-one knew about the paperwork. They said they would investigate and call me back. They never did. I called them and was told they knew nothing but it would *********** months to fill out the paperwork out once they found it.. They would call me back. 2 months later still waiting.
