I am reporting this facility so that what happened with our family and our loved one will not happen to other families. During COVID there was no way to contact nurses to learn about my mothers well being or to contact my mother. I sent letters weekly and when I finally was able to enter the building all of my letters were in her drawer unopened. No one read them to her or gave them to her. The facility has very poor communication. WE were never able to talk to the director or nurses on the phone. Bad phone service, no messages are returned, unable to talk with my mother on the phone because of the phone service. If I emailed the director she would respond politely just to placate me to no effort to change things. I finally requested a family meeting with the director, nurses, head nurse over drugs and the physical therapist. This was done over the phone. They just repeated everything exactly the same as before Covid or since 2019. There were no programs, no PT, no interaction with others during Covid. I asked if my mother had a stroke because she displayed physical characteristics of one and they did not know if she did. October 2021 I was called by a nurse to say that my mother was unresponsive. This nurse was anxious, almost hysterical. She said she gave my mother her medicine and then her heart rate went off the charts and then she was unresponsive. She called the doctor and said she would call me back. No call back whatsoever. I could not contact ANYONE because of the phone service. At this point I called hospice because I knew they would be there and be responsive. The centers doctor called me and said he was on his was would check on her and call me back in 10 minutes. HE NEVER CALLED ME BACK! I live 5 hours away. I had my 2 brothers go over to the center and they said no one knew what was going on. After her death 2-3 days later I cleaned out her room. I called every head of dept, notes, mess. Then 2 wks later hskping called asked wht
