I went to see **************** on 2/12 to establish care and receive my annual visit. She does not have any of my past medical history or anything. We talked about some pain i was having in my L knee and lower back and i requested medical records. We discussed diet, exercise, and i requested blood work and additional testing as i do with every visit. That was it, she didnt take any notes while in the visit or anything. She didnt have a lab top for notes or anything. She then includes a diagnosis on my chart that was incorrect and we NEVER discussed this. On top of that she then included another diagnosis that was incorrect. **************** has caused distress and shown incorrect coding and billing practices. She hasnt followed up with me on my labs results or any results for that matter. *** reached out to her office numerous of times to address this with little to no care in response time. I was told an investigation was being conducted in regard to the first incident that occurred, but I havent heard anything since. I was told it would take a week and it has surpassed that. I just found out about the new diagnosis today from my insurance as it is not showing on my records when i review the patient portal which again draws more concern. Because why is something being billed to the insurance but not showing up on my records in the patient portal? This office lacks communication and is doing whatever they see fit when it comes to billing and coding and in turn it is causing negative affects on the patients especially to say they have something when they do not, that is a horrible thing to do. My concerns should be taken seriously and i need this matter addressed as soon as possible and a thorough investigation, coaching, and training rendered to not only this provider but her entire care team. They have to do better.
