On March 19, 2023 Medshield Labs LLC, **************************************************************************** presented a bill to ********* Claim #**-23135-775-810 in the amount of $120.00 for providing self-adminstered Covid-19 tests. They were paid $94.08 by ********* These tests were not ordered, nor were they ever received. On April 1, 2023 again Medshield Labs LLC, **************************************************************************** presented a bill to ********* Claim #**-23136-869-270 in the amount of $120.00 for providing self-administered Covid-19 tests. Again, they were paid $94.08 by ********* Again, these tests were not ordered, nor were they ever received. There was no knowlege of this activity until receiving and reviewing the ******** Summary Notice received today September 9, 2023. There have been 8 previous bogus *************** services theat have done the same thing to myself and my wife. Billing and receiving payment for services not solicited or received without approval or knowledge. Multiple complaints have been filed with the ************************* at HHS. They will be updated with these new events to bring the list current. Something must be done to stop these people. Nothing has been paid by me or my wife directly but our ******** accounts have been hit repeatedly.Initial Complaint

Medshield Labs LLC billed ******** ****** for a covid test that was never done. In fact I do not live in the same State this company is listed to be in and have never used their services. ******** reimbursed them $94.08 for the test. This is outright fraud against my insurance company and I want to see them reimbursed in full. I attempted to call Medshield Labs LLC today and the phone goes straight to a voice mail. Fraudulent claim was submitted by this company to ******** for a covid test I never took. Claim date was 03.22.2022. I called ******** to report this as I had no doctor visit, no labwork done in March. I have never ever received, taken, nor submitted a covid test. Today, July 10, 2023, I received an HOP statement that indicates Medshield Labs LLC in ******, ***** billed ******** B a charge of $120.00 for a Covid test (purportedly for me), with a date of ******* of ***** 22, 2023. I have never had a Covid test. I was not in any lab for testing on ***** 22. Nothing of mine ever was sent by any medical facility to Medshield Labs. This is the second time this year that a purported "Lab" in ***** (the other one being Ipseity Diagnostics in *******) has made a fraudulent claim regarding Covid-related *******s and fraudulently used my personal ******** information to submit a bill. I have reported both frauds to ******** ** Tiers I and II and to HOP.Customer Answer

I have learned from ********* Tier II, that this **** is in reference to an "at-home Covid-19 test kit." Because I never ordered a test kit from this lab and in fact never received a product of any sort from this company, MedShield's fraudulent use of my ******** number is referred to as "Phantom Billing." A string of "labs" in ***** and in ******** all have used the same ruse (i.e. phantom ****ing for at-home Covid test kits) to extract payments from ********. Some of the labs are back-dating their statements in order to continue to **** ******** past the end date of May 12, 2023, for ****ing for home test kits. ******** paid all of the ****s, with the exception of MedShield's. I have followed through with reporting each and every instance of phantom ****ing to Tier II at ********* and all are being investigated. I have been granted a new ******** number to try to prevent future fraudulent use of my personal account. I do not know how ******** will recover any funds from this apparent crime ring. I have learned that ******** has received numerous fraud reports for phantom ****ing/phantom Covid test kits from "labs" such as the ones that have ****ed ******** by using my account information. Thank you for your efforts on my behalf.
