    On 01/03/2022 I was told that I needed a tooth extraction and that the procedure fees would be $854, with an estimated out-of-pocket expense of roughly $210.80, because my insurance covered this procedure. The procedure took place on 05/17/2022 and an initial fee of $180.40 was paid. The dentist submitted a claim to ***** ****** insurance on 05/18/2022 for $1,309 (significantly more than was discussed). ***** ****** refused to cover part of the procedure according to the codes used on the claim and because the extracted tooth number was not included in the claim. The dentist resubmitted the claim on 06/24/2022 with different codes and again ***** ****** refused to cover a significant portion of the claim. In total, ***** ****** only covered $100 of the claim. I received a payment statement from the dentist, sent on 7/15/2022 for $728.80, with no specifics regarding what I was paying for or why it was nearly 3.5x the estimate. I called ***** ****** to get an understanding of the Explanation of Benefits (EOB) and was told that oral surgery should have been included in the extraction, but if the dentist would provide written justification for why this was necessary in addition to the extraction, it would be reviewed and perhaps approved. I spoke with the dentist's billing manager about this requesting their assistance. The billing manager agreed to speak with ***** ******, but said that the dentist refused to submit a revision or provide a justification for how the procedure was claimed and billed. I was told that they would give me a $100 discount, but otherwise would be unwilling to provide additional assistance vis-à-vis the insurance company. I received an update bill on 8/18/2022 for $628.80 that again did not include specifics regarding what I was paying for. I called the dentistry on 8/19/2022 asking for a breakdown of the bill. A breakdown of the charges was received on 8/25/2022.
