    Palmetto Podiatry provided services to me on 8/15/2022. At that time, Palmetto Podiatry required prepayment of $395.00, pending insurance reimbursement.
    My insurance, **** ********** ****** made payment to Palmetto Podiatry on 10/11/2022 via claim #*********** in the amount of $246.40.
    This amount represents payment in full in accordance with Palmetto Podiatry's contractual agreement with **** ********** ******.
    I sent a letter to Palmetto Podiatry on December 18, 2022, along with a copy of the Explanation of Benefits.
    I asked that Palmetto Podiatry refund the overpayment of $395.00.
    I did not receive a response to letter.
    I called Palmetto Podiatry and left a voicemail for the office manager on January. 30, 2023 . I did not receive a call back. I sent a message through the Palmetto Podiatry website and did not get a response, I called again on February 1, 2023 and left another message for the office manager, who did not respond.
