              On March 23, 2021, I requested a recheck of my last eye exam performed by Sewickley Eye Group - this was done by Dr. V at Monaca office; re-exam cost was $100 out of pocket & I ordered eyeglasses and frames. Cost to me after my eye insurance was $232. On April 12, 2021 I attempted pickup & fitting of the completed eyeglasses. I found that I could not see through them, refused to accept them, and directed them be sent to the main Sewickley Eye office for reevaluation. After an additional re-exam there by Dr. V during during which an astigmatism error was inadvertently made on RX by her, new glasses were made to the error RX; as later found the right bifocal was also incorrectly located by supplier. Please see additional supporting documents - Refund of my additional (third) re-exam and second set of lenses made to replace the ones made in error is $327 out of pocket and is requested - I will return the error Sewickley's lenses as well. (See enclosure for more details.)
