      On April 17, 2024 I was examined by Dr. ********************** for prescription lenses. AS soon as the examination was over I met with the optician to order lenses. Before anything the optician asked for my ***** **************** card and then to my astonishment asked for my medical insurance, a service which was not provided by Dr. **********************. Having respect for my community, I complied. she then led me to a desk to make measurements and fitting glasses. Before I knew it I saw a bill for $1127.00. Again, I have been living in this community for over four decades and I have deep respect for my fellow ciiizens. I gave her my card and was charged. I left to go to work at the **********************. On the way I realized that this charge is way excessive and basically I have been taken. I call the office and asked for a refund. Again,the optician answered the phone and I requested a refund. To my astonishment, she replied that the lenses have been ordered and a refund cannot be issued but she said she would discuss with Dr. **********************. She dis consult and informed me that no, the lenses have been ordered and no refund can be issued.I am a highly myopic patient and in the interest of no delays so I can go about my business which is vision dependent every second. After several days, I called and I was informed that the glasses were not ready yet. I emailed the Dr. and explained that making glasses does not take more than 3 or 4 days. This is a medical device and its timely delivery is essential. Dr. ********************** cam up with yet another after the fact excuse that it takes between 10 - 14 business days. These are all excuses: from the ordering of the lenses at the speed of light and the delivery of a urgently needed medical device at the buggy speed, this whole scheme is designed to usurp people's money to their benefit. In the attached receipt you see that they threaten patients with collection agency and **** fee penalty but the dispensation time is left blank. Their conduct cannot be explained in any pother wasy except taking advantage of patients. In all of my sixty years of wearing eyeglass, there has never been an optometrist on whose receipt there is the threat of collection agency, 1) I am asking first for them to dispose of my medical insurance because under no circumstances they are entitled to it. I am not sure how unlawful it is to collect medical information when one is not entitled to it. 2) I have already placed a stop on the credit card and **************** has found my grievances to be valid.       I had an office visit at Domiano Eye Care in Old Forge PA on 1-10-23. This was for my fully covered annual diabetic eye exam. Prior to seing the eye doctor I was asked to sign a large stack of slightly blurred forms. My current glasses I was wearing were not allowing me to see very well as this was the reason I was visiting the eye doctor. I was then asked if I wanted an "extra, deep eye exam" for an additional $39.95 (I declined it) or just the regular diabetic eye exam covered under my health insurance. "I said I would only approve the eye exam that was fully covered under my eye insurance and with no extra out of pocket expense". The said they understand and we all agreed to conduct the exam.
      I completed the exam which included a "visual field examination" (a procedure I have never had previously, anywhere). My bill for the glasses was more than $700 and I declined to purchase the glasses based on the extreme cost. The exam was then over and I left.
      On 2-3-23 Domiano Eye Care called me and said I owed them a $30 co-pay that for the "visual field examination". This was my insurance co-pay for that particular part of my eye exam.
      I never approved this particular exam as I told them when I first entered the office, "no procedures that are not FULLY covered under my insurance plan and no out of pocket expenses".
      I was never given the chance to decline this particular part of the exam like I was given a chance to decline the "extra, deep eye exam" which I did decline.
      Domiano Eye Care NEVER asked me if I wanted to decline this "visual field examination", and performed this exam without my consent and are attempting to charge me a $30 co-pay for an exam which I would have declined if they gave me the opportunity to decline.
      Domiano Eye Care insists that I agreed to every and all exam procedures when I signed the stack of blurry papers an I owe them $30. Obviously I legitimately disagree.

      I am rejecting this response because: 
       ******* ******** response is detailed but inaccurate and distorted, she is incorrect and convenietly leaves out critical information supporting my claim. 

      #1-My annual diabetic eye exam was not paid by my medical ********* medical insurance, my ********* medical insurance never paid for my eye exam. It was fully covered and paid for by my *** eye insurance as a fully covered annual eye exam. No one from her office ever informed me about the difference between the medical exams and the eye exams.
      #2-I did have difficulty clearly reading the stack of papers because of my diminished vision. That is the exact reason I was at the eye care center to get examed for a new pair of glasses because my vision has changed and is affecting my reading and distance vision. She confirmed herself that my vision has changed and declined.
      #3- I did decline the "optical coherence tomography exam" specifically because they told me that my medical and vision insurance did not cover it. This is my whole point of  Domiano giving me an opportunity to decline a not covered exam for this particular exam but not offering the same decline option on the "visual field exam".
      #4-This is not about my previous eye care doctors and their performing or lack of performing a visual field exam on me. This is about Domiano preforming this visual field exam which is not a fully covered exam under my ********* medical insurance, there is a $30 copay, and that is the ENTIRE reason for this dispute because Domiano went against my specific request to NOT run a exam that requires any copays by me.
      #5-The original total bill for my eyeglasses was ABSOLUTELY around $700, after my $200 frame allowence was applied, but I declined that offer due to the high expense. The doctor then offered me another option of around $500 with the same $200 allowence applied, but I declined that as well due to the expensive cost. She said she understood and that conversation was over.
      #6- Yes I stated that I wanted only procedures that were fully covered under my "*** eye insurance plan". No mention from their staff about any copays with my "********* medical insurance plan".
      This was a supposed to be a fully covered eye exam with my *** eye insurance plan not a medical eye exam.
      #7-I was never given a chance to decline the "visual field exam", but I was given a chance to decline the "tomography exam" and I was also given the the chance to decline the two different eyewear price options. This is my whole point of contention with this complaint. Domiano should have honored their verbal agreement to give my the opportunity to decline any exam that was not fully covered by my insurance like they did previously with the tomography exam and the extreme cost of their lenses and frames.

      Domiano tried to charge me $40 for a "speciality office visit" under my ********* medical plan. At the time of my visit I paid this fee with my credit card. Later at home I checked with my ********* medical insurance representative and I was told that my visit to Domiano was fully covered under my *** eye insurance and was not considered a "speciality office visit" and therefore was NO  charge for a "speciality office visit'. I then disputed the $40 credit card charge based on the facts given to me by my ********* health insurance representative and my credit card company upheld the dispute. It was my understanding that later that Domiano's claim for a "speciality office visit" was denied by my ********* medical insurance., because it was NOT a speciality office visit as I was told by *********. So the Domiano charge of $40 was incorrect! I'm curious why Domiano neglected to leave this fact out of her response?

      I did not expect to pay anything for my fully covered diabetic eye exam with my *** eye insurance plan, which was always the case at my two other eye doctors I visited over the past years under the exact same plans. Instead Domiano treated this visit as a medical exam and subsequently tried to charge me for medical diagnostic tests, without my permission, under my ********* medical insurance along with an exorbitant price for lenses and frames.

      My issue IS with the copay for a diagnostic procedure BECAUSE I specifically stated to Domiano that I would not authorize any procedures or exams that involve a copay BUT they preformed those on me anyway.

      Domiano did get reimbursed for my eye exam from my *** eye insurance and also reimbursed for the non-authorized visual field exam from my ********* medical insurance. The $30 copay is payable from me to Domiano and not to my ********* medical insurance. Therefore Domiano should dismiss the $30 copay based on my specifically requested non-authorization of the visual field exam. 

      I recieved a copy of the prescription from Domiano for my eyes and used it at another eye doctor and there was no copay, and no additional charges for similar lenses and frames.

      I am the victim in this case because I am being charged for something I never approved. Domiano has tried to turn this entire, unfortunate incident into my fault.
      This further reinforces my contention about my dissatisfaction about my entirely unacceptable Domiano visit.

      Furthermore, I am extremely dissappointed in the inaccurate responses by Domiano Eye care which were partially irrelevant, incorrect and distorted to contradict all of my factual information. 

      Domiano Eye Care has referred to themselves as a "Premier Optical Store" with "unique eyeglasses hancrafted from the finest italian materials". I cannot say for sure who, but someone has to pay the expenses of this high end vision provider ? and it will not be me.
