    I was receiving physical therapy from the accused for aprox 6 months. Once I stopped going they continued to schedule appointments that I didn't ask to have scheduled. Now they are attempting to collect cancelation fees and threatening to send them to collections.

    The patient did recieve PT for approx 6 months. The patient came in on 3/11/25 for an appointment and scheduled his appointments for the next two weeks. We do not schedule the patients on our own they are here and set up the appointments. We did bill the patient for 4 cx fees totaling $100. We were being very understanding considering the patient cancelled and no showed 21 times and we only charged him for 4. They patient signed a form making him aware of our no show and cancelation policy. (see attached)We attempped to rach out to the patient when he no showed appointments but his mailbox was always full. (see attatchment). We did have to discharge him due to his non-attendence compliance which you will see in the therapist final note.
