      Cognitive Behavior Institute charged $182 to our FSA card. It should have only been $40 (copay). The insurance company's Explanation of Benefits (EOB) form explains that the charge should only have been $40. I called Cognitive Behavior Institute 5 times once the error was caught. I get their answering service and left numerous messages asking for a call back. The merchant will NOT return calls. I have asked for the $142 to be refunded, but they do not respond. I am now out the $142 and have had my FSA card frozen because I cannot get a hold of ANYONE at Cognitive Behavior Institute office.
