    My partner bought this company’s life insurance policy through a salesman. The salesman did this over the phone. The salesman put No to the question about smoking, COPD, etc. At the time my partner had quit smoking. He stopped in 2018. So my partner pays for this insurance almost 2 years. He has since passed away from pancreatic cancer. It was very quick. He died within a month. The insurance company refuses to pay because they said my partner lied on the application. They do not insure anyone who had smoked or had COPD. The person who sold us the policy knew he smoked. The company said they sent my partner the policy and he had time to look it over. They never sent a policy. I have not found a policy from this company anywhere. The policy was signed electronically and that should be illegal. They should have sent the policy for his signature. They sent me a copy of that policy and told me why it was refused. ****** C ****** sold him a policy prior to that and told him about this one that was cheaper. My partner canceled the other insurance and started with Life Shield. The only reason I knew which company it was was, was because I seen the deduction from his checking account. There was no policy sent here to go over. We had no idea this company did not accept any smokers. ****** is at fault for selling him this policy and the company is at fault for not sending the insurance policy and getting my partners signature. Can I get retribution for their errors? I want the $5000.00 he was told we would receive. My partners name is ******* A *****.

    Due to HIPPA and privacy laws we are not able to provide specific detailed responses to this complaint.  However, the life insurance policy in question was issued
    based on information provided during the application process, which was
    completed and signed electronically. Electronic signatures are legally valid and meet all state and federal guidelines. Every policy is mailed out to the address provided on an application, the policy was not returned.  Their is a free look period and the insured is advised to review all of the answers on the application and advise us of any discrepancies.  There was no response that any answer was incorrect. Additionally, we did not receive any
    calls or complaints from the insured during the time the policy was active that
    a policy was not received.

    At this time, based on the available
    documentation, our position remains that the claim is not payable and premiums will be refunded, due to
    material misrepresentation on the application.

    That said, the beneficiary does have the
    right to appeal the decision. To do so, she would need to submit a written
    letter of appeal to the Company that includes:
    Policy number:
    Insured’s name:
    A clear statement that she is appealing the denial
    An explanation of why she believes the claim should be
    reconsidered
    Any supporting documents.

    Legal Shield said they would save me money and that I should cancel my policy with United of Omaha. I was told that I would get a payment back from my bank that I paid to them and that it would be 95.52.

    Life Shield National took money from my account and had Mutual of Omaha cancel account with MO.  I did not cancel my Mutual of Omaha insurance; they did.

    We received ******* ****** application on 3/7/2025 at which time the policy was mailed. ******* *****s called LifeShield with the writing agent on 3/18/25.

    She stated she never approved this policy and also noticed several of the questions on the application were answered incorrectly. She did state she is a diabetic, smoker, and has COPD. She then requested we cancel the policy. We changed the status on the policy to not taken and mailed a letter to ******* *****s. Our records show that we did not draft any premiums from her account. The agent may have submitted applications to other companies. 

    At the end of her complaint, she states that money was withdrawn from Senior Life Ins. LifeShield is not associated with this company. 

    As of now, our records still show ******* *****s policy as not taken and no premiums received. 

    We will reach out to ******* *****s to clarify the above and see if there is any other way we can assist her.
