    On January 9, 2024, Dr. ****** ********** of Northwest Animal Hospital made a significant mistake with communicating the correct prescription for a liquid compound of ********* to *** Pharmacy/OptioRx, and then Dr. ********** intentionally lied about knowing who made the mistake on January 16, 2024 and on March 2, 2024. The dosage directions on rx 432075 read, “Give 0.5 ML by mouth twice daily for 10 days, then 0.38ML twice daily for 10 days, then 0.25 ML twice daily for 10 days. The correct dosage given to Dr. ********* from ****** was 0.5ML two (2) times 10 days apart, then 0.38ML two (2) times 10 days apart, then 0.25ML two (2) times 10 days apart. Instead of being ethical and taking responsibility for her significant mistake with *******’s liquid compound ********* dosage, she chose to send me a text instead of calling me and lied to intentionally mislead me to believe that ***/OptioRX made the mistake. On January 16, 2024, Dr. ********** texted me “This is Dr. **********, This label is definitely wrong! I’m not sure what happened in the translation from us to the pharmacy ... . There was an issue with my ordering Revolution Plus at the end of February. I again commented on the mistake with the ********* and said I was going to change vets. Then on March 2, 2024 in her office in front of a male staff member, Dr. ********** lied to me when she said she did not know who made the mistake. I called ******* March 4, 2024 and the person on the phone shared that their records show that ***** **** from Northwest Animal Hospital called in the ********* prescription on January 9th and spoke directly with a pharmacist. Once ***** verbally stated the prescription with the wrong dosage, which had to be provided to him by Dr. **********, the pharmacist read it back to ***** and he was asked if that was correct, which he did. Had I not caught Dr. **********’s significant error, ******* could have quickly developed serious health issues from too high of a dosage of *********.
