            I am having a horrible time trying to resolve an issue with this company, they are trying to state that I never returned a form and I never received it, and am being charged $1,000 a year on top of higher premiums for my health insurance. I have called them several times and they refuse to work with me. The most horrible part of all is that the company now has Tabacco Surcharge on my paystub making it impossible to rent a smoke-free apartment in NYC. UGGG. Now the fact that they have it in print on my pay stub makes this illegal. it's; deformation of charter. I am giving them one last opportunity to correct this before I file a lawsuit in NYC. there is no reasoning with this company and logic seems to not valid.
