      Hello, I visited the CityMD at *******, **, location where I provided my insurance details at the counter. The front desk told me my visit would be fully covered by insurance. Later I get a bill of $300 saying that this location is out of network and insurance doesn't cover. I'm requesting the amount to be written off as I was miss-informed by the front desk. Additionally, I did not get any treatment for my ailment - no prescription, no resolution or any sort of help from the doctor. Requesting my bill be adjusted to zero. Or another option for you is to submit the claim under a CityMD location that is in network.

      I went to city md on ******* after an injury happened at work, the doctor was very rude and treated me like Im trying to get time off from work instead of actually treat and check me, she told me that Im good to go after 10 minutes and sent me home. I went back to work which probably made my situation worse. I suffer from hearing loss, neurological problems and a concussion that doesnt go away.

      CityMD told me that they do not accept my health insurance when I came in there on October 22, 2022 with a head injury. I was told that since it is a head injury they are required to take a look at my head. I asked if I will be charged anything and was told "no." After a worker from CityMD took a look at my head I told to go to the hospital like I said I would in the first place. Almost a year later, instead of sending me a bill, I receive a letter from collections agency that I was placed in collections by CityMD of *******, ** I did call the billing department of CityMD and charges were removed from their end but the workers are refusing to take me out of collections department saying that I can just sent a bill that was emailed to me that shows I owe CityMD zero dollars. When I call collections department and tell them that I have a print out from CityMD that I no longer owe CityMD money and would like to send the collections that copy, I am told that CityMD needs to do so but CityMD consistently refuses to do so saying that I should just send the letter with zero balance on it and it is sufficient enough for me to be removed from collections when in fact it is not

      I visited CityMD **** **** *********** on 8/25/2023 for some blood work. Before checking anything, I specifically asked admin at front how much I need to pay for the visit. After a long time checking my insurance coverage, she said I DO NOT need to pay anything for this visit. I also double confirmed with her that it means $0 payment for me. I got 2 bills later, one for visit and one for the lab testing (in total $300). If I was told I had to pay anything for the visit, I would not go there at all.

      : Account number *************** Bill ID ************** I am disputing the bill in the amount for $50.00 for services which I did not receive. I personally disputed this bill and verbally disputed this bill on at least three prior occasions.On June 5, 2023, I went to City MD Ozone Park Urgent, *********************************************************** to seek an Ultrasound on my lower left leg. I specifically inquired with the receptionist whether this location possessed the Equipment to perform the ultrasound. I specifically inquired about this because I previously was diagnosed with a partial DVT in that same leg. Both the receptionist and Assistant Manager, *********************, instructed me that I had to speak with the Dr. about whether the facility possessed this equipment. I informed both that I do not need to see the Dr. if the facility does not possess the necessary ultrasound machinery to perform the test on my leg. I further stated that I do not want to pay for something that the facility cannot provide. I was again told that I still had to see the Dr. since only the Dr. can tell me whether they have the equipment. The receptionist then told me I had to pay my $100 ***payment prior to seeing the ************** this instruction, I paid the ***payment.Shortly thereafter, I was called into an examination room, glanced at my calf and stated yes, it's swollen - you need to go to an Emergency Room since we do not have the equipment to perform an untrasound.This service lasted 45 seconds to one minute. I complained to ************** about the non-service I received. I felt that it was disingenuous for the CityMD to request my ***payment upfront when ************** was aware of the specific type of service I requested. A service that CityMD does not provide. ************** subsequently canceled the $100.00 ***pay. After leaving the facility, I called my ************************ and informed them not to pay any bill for services that the CityMD was aware it could not provide.

      Prior to receiving medical attention and while the administrators were reviewing and filing insurance information, I stated explicitly that I wanted to consider the personal cost of the visit and would need advance notice of cost of medical attention and treatment. I was assured by the administrator behind the desk that the insurance company covered the entire visit and my copay *prior to entering the examination room* was $80. I paid this amount in advance of medical attention and was then not made aware of any additional charges until I received two bills in the mail that totaled $166.77 (the amount of money claimed in this claim). I have called and spoken with CityMD billing only to be rejected and dismissed with this complaint and request for restitution. I have notified CityMD billing several times that this is near fraudulent activity that abuses my credit and is not justified. The services rendered were excellent but this billing practice is unjust especially with my advance request and with my insurance covering the significant majority of the bill. Please help me Better Business Bureau I gave CityMD advanced notice that I wanted to have full awareness of the cost of my visit and they gave me their assurance that they are now going back on and I'm the one that has to pay for their mistake.

      I visited CityMD on July 27 after cutting my finger and was seen by *********************************. I was told to keep my finger permanently bent in a finger brace I was given (apparently so I wouldn't lose range of motion), and for a week I diligently followed instructions. After I checked in again, I was told that my finger would heal back permanently bent, since I had a ruptured tendon. Because of the faulty advice I was given, now the tip of my finger is permanently bent for the rest of my life.

      I went into CityMD for STD testing on 5/6/22 and was seen by a medical assistant who asked me some questions, did my vitals, **** my blood and then said I'm good to go unless I had any questions for the doctor. Shortly after, insurance covered testing but I received a separate doctor's **** for $300, when I did not physically see a doctor or PA/NP. I refuted the charge but CityMD still charged me and sent my **** to collections. On 2/10/22 and 3/14/22 I also went to CityMD for routine STD testing, saw a doctor for 30 seconds on each visit and they up****ed the charge to a ***** - 15 min visit for seeing a doctor for a "hi and bye" visit. They again attempted to charge me $300 for each visit which is seems to be up****ing.

      Someone from CityMD contacted me to explain the billing codes for 15 minute visit. They had no response for the fact that they billed me a physician bill when I did not see a provider on my last visit in May. They state they were going to speak to the medical directors and get back to me. They still have not contacted me in this regard, I have already previously taken this up with patient relations with an inadequate response.

      To Whom It May *************** multiple requests, there are no visit summaries or X-rays or medications are available in my patient portal for my most recent visits to **** ***** ** and **** ***** ** for a toe accident in May 2023. I called the **** ***** branch and ***** branch multiple times after being transferred to their ******************** since none of the employees on site were able to assist me further, even though they could see my medical records in their system. Despite choosing the medical records option several times, the phone system failed to register the automated voice recording kept repeating in a loop. ***** at west ******** then provided me the direct phone number ************ with extension **** which she said to enter after prompted. I called the number, no such prompt. I entered the extension, no effect. I chose option 5 for the medical records department and ended up being transferred to billing. The male representative who answered repeated that I had to choose extension ****. I told him it didnt work and he transferred me to another line where I was put on hold for 30 min before the line automatically disconnected and I was hung up on. I have multiple recordings of all the aforementioned phone calls.

      Fraudulent billing. City MD not only accepted a patient using stolen medical insurance information but is also playing run around and not taking the matter serious. Whether knowingly or not, they either provided services to an imposter or made an egregious billing mistake that they have yet to remedy.

Billing is only one part of this complaint - this company is refusing to provide information, my information, that was stolen and used to obtain illegal medical services. I have had to place things on-hold due to this matter and I am becoming increasingly frustrated in the manner this complaint is being handled. The next step for me if my case is not resolved to my satisfaction (billing repealed, comprehensive report as to how this occured and what information was compromised, medical record correction, to name a few), I will seek other means of resolving this matter.

      I'm not getting concrete resolution from CityMD/Summit Health - the ******** of ******** Affairs has been 100 times more helpful than CityMD - at least the ** government agency is trying to assist and help me. CityMD have yet to provide the information I have requested regarding this case AND they rarely update me despite several emails I have sent. They need to do better with this. Instead of making me, the victim, go on a wild *********** maybe they can show some proactiveness and contact me instead of me having to send multiple emails to get borderline non-answers. Inexcusable way to conduct this case. All I get from CityMD is that they are actively working on this....actively working on something is NOT a resolution. The fact that they are so unwilling to provide the information (my information mind you) regarding the identity theft is absurd and likely illegal.
