      Date Of Transaction: August 1,2023 Amount Paid: $1,260.00 Receipt:#***** I was due for an eye exam. I paid a total of $1,260 for a pair of frames. The receptionist did not provide me with a breakdown of what my insurance covers nor did she discuss the difference in what I would be paying out of pocket. In addition, I brought a second pair of frames, and I told the receptionist that I rather them put the prescription in the new frames that I bought from Cartier... instead of the ones that I purchased at their location. They agreed because they do not have a refund policy but store credit. Later that week she stated that I can no longer get store credit. I need my money back and I need my glasses back.

      I was seen at Family Optometry 121, P.C., for an eye exam and to place an order for contact lenses, the first week of December 2022. I paid-in-full for the/my contact lenses at the end of the appointment--$200 for a three-month supply of lenses. Two weeks+ later, after I still had not heard from the office, I went to the office, in person. The office's reception staff told me that my lenses had not arrived, due to the fact that there was a "supply problem" with the vendor. I was given 15 contact lenses, i.e., a one week supply, and I was told that I would be contacted when my order arrived. A week later, since I had not heard from the practice, I went back to the office. Again, I was told that my order had not arrived and this time, I was offered another week's supply of lenses--in a different/lower prescription! Since, I had no other option, at that time, I accepted this temporary--and altogether questionable!--option. At that time, I also asked to speak with the practice's Manager--"******." ****** and I spoke in-person and he promised to follow-up by speaking with the vendor about the delay. ****** called me a few days later and said that the vendor was giving him "a run-around," but that he expected the order to be delivered in the coming days. Despite this claim, the order still did not arrive and ****** told me that the vendor was still making excuses. I called the office on Wednesday, 25 January 2023--almost two months after the order was placed!--and I was told that the/my order still had not arrived and that the office manager, ******, was not in the office and that they did not know when he was going to return. I called the office the next day/Thursday, January 26th, and I asked for the name and contact # of the vendor. Uncannily, an hour later, I was told that my order just arrived; I was given one box/90 lenses and 15 loose lenses, instead of what I should have been given and paid for--two boxes--90 lenses each--of contact lenses!

      My name is **************************************** , me and my children have been going to this eye doctor for 2 1/5 years . My chief complaint is that my son ***************************** who is medical necessay have not been getting his glasses , i spoke to the employee name (***) name of the person that is the business manager., and she stated that they are waitng on the insurance to send pernment contacts and medical necessecy glasses. my son started wearing glasses when he was 5 years old. I take the advice of ************ and allowed our son to get contacts. I was told that he can get contacts and glasses due to his age, he is still waiting for these items. I was told they was only going to give him temporary contacts until his permanent contacts was issued. My son stated wearing the contacts and was only given one a day boxs of lens and two weeks . He still havent received his glasses it has been over two months. i called ******** healthfirst they stated that the office received both contacts and glasses.
