- Cancellation of Surgery/Non-Approval of Estimate: If the customer decides to not have the surgery performed and their patient is here, there will be a $50 fee for the office visit and $25 for return shipping. The patient will be shipped back via UPS with tracking and $500 insurance. The customer will receive an emailed UPS tracking confirmation in order to track the package. Regardless of whether your patient was here for the initial estimate or you sent in photos, the estimate fee of $30 is non-refundable.
