    Dr. David O. schedule my annual Lung Cancer Screening Test which is part of **** preventative screening process
    I am screened every year
    I was informed that as part of the Affordable Care Act preventative screening is covered 100%
    Which has been the case the last three years I have had my screening.
    I went to this facility as directed by my health care provider who made the appointment on my behalf as they always do
    I received the results and followed up with my health care provider
    Again I was told this procedure was fully covered
    I received a bill in the amount of $375.95 from ****
    I phoned the billing office and they said "We all pay alot for health insurance, your test was not coded properly as a Wellness DXL, you have to pay"
    I explained in 7 different phone calls to my insurance company
    the providers office, the hospital and the billing department this is covered as preventative and I should not be charged
    I was told "don't pay it and see if they come after you"
    "Pay it or we will report you to a collection attorney and ruin your credit"
    "Set Up a payment plan"
    I was also told "You should have gone to an independent facility, that would be paid 100%" I explained I did not know this was not an independent facility and my health care provider made the appointment for me or I would have gone elsewhere.
    I am requesting this bill be deleted as it is not owed.
