    I am filing this complaint to warn the public of serious concerns regarding patient care, transparency, and documentation practices at Dynamic Dental *********. During my 11/05/2025 visit, the dentist did not introduce herself, did not explain the procedure, and did not review X-rays or any treatment plan with me. At a previous appointment, I was told one tooth would be monitored (watch). Without discussion or consent, three fillings were performed. No explanation was given for this change, and no one had documented or reviewed any fillings with me in over a year. The care provided was substandard. The fillings were left rough and unpolished, making it painful to chew. I had to return the same day for correction, and this impacted prior dental work completed elsewhere. A paper authorization copy was only given after the procedure, and it was never explained. I asked questions and was ignored. I was told I would receive a copy later, and instead, **handwritten additions appeared on the version placed in my file after the fact When I told the office manager that the form had not been signed in front of me and I never received a copy at signing, I was ignored. This raises major concerns about altering documents after a patient signs. No X-rays were taken or reviewed for over a year, and the records I received were incomplete. The X-rays provided contained no name, date, or tooth numbers, which raises billing and documentation concerns, as records must support the treatment performed. When I attempted to address the issues, I was spoken to dismissively, and my concerns were not taken seriously. Combined with constant staff turnover and communication issues, this environment is not consistent or safe for patients. My full complaint is attached. I authorize Dynamic Dental ********* to speak with the BBB regarding this matter.

    I am rejecting this response because: Their statement does not address the issues I raised, does not respond to the specific requests I made, and does not match the evidence in my possession.
    Dynamic Dental has refused to provide full and compliant documentation. Required records, including properly identified radiographs, unaltered consent forms, complete ******** clinical notes, CDT-coded billing records, and the full treatment-plan history, were never provided. Their narrative to the BBB conflicts with their own chart entries and with their prior emails and statements.
    I have recorded calls, in-office recordings, and written communications from Dynamic Dental confirming that at my June appointment I was on limited time, which directly contradicts their claim that I declined a treatment-plan review. Their internal documentation has changed across versions, and I have the original materials showing missing entries, inconsistent notes, and handwriting added to forms after I left the office. Dynamic Dentals response to the BBB is not accurate, does not reflect the clinical record, and does not address the specific regulatory concerns I raised.
    For the BBBs awareness, a formal complaint was submitted to the ************************** of Dental Examiners on 11/12, and both the dentist and the practice have been notified. Because the matter is now under active state investigation, I do not expect the provider to be fully transparent in a public BBB exchange. The Board and other regulatory authorities are receiving the full evidence package, including recordings, documentation comparisons, and email records that support my statements. I am closing the BBB complaint only because the matter is now in formal regulatory review, not because the businesss response was acceptable. I reserve all rights and remedies.
