      I, would like to file a Complaint against ***** London of London Disability for DISABILITY FRAUD, False Statement, Concealing Information, Using or Presenting False Documents, FORGERY of a Document, and Financial Exploitation of a Disabled person, Violation of my Civil rights A. Violations: Forgery of document, False Statement, Financial Exploitation of a Disabled Person, Fraud I contacted London Disability around 5/18/22 seeking representation for my Social Security Disability appeal hearing 7/6/22. I was alerted that SSA received a signed Standard Fee Agreement, dated 5/25/22 signed by both attorney & claimant was faxed 6/3/2022. The issue is my Initials was FORGED on this document. This document was received on 6/7/22 via email from Jasmine ******* of London Disability with attached SSA Form -1696 and the Standard Fee Agreement. (Can provide email verification). On 6/14/22 @ 10:56am the Form-1696 Claimant Representative Form ONLY, signed date of 6/13/22 (Not a Signed Standard Fee Agreement) was sent via email to Jasmine Burnett. I didn't sign that document because I didn't understand it. On 6/20/22 @3:18pm a follow up email was sent to Jasmine Burnett to confirm receipt of the Form-1696.Received a response 6/20/22@3:33pm. B. Violation: False Statement, Fraud, Violations of my Civil Rights, ADA On, 9/7/22 received a copy of a letter/ attached frSSA-1560 Claimant for Authorization to Charge and Collect a Fee for Services Before the SSA from Scott ****** & Jonathan ****** submitted to Honorable Tara ******** I have no dispute of the Fee amount requested by Jonathan ******* Scott ****** alleged that services began 4/20/22 to 7/27/22, with the Fee amount request of $3393.52 providing a total of 401 minutes(6hours & 41minutes of service. ) I'm disputing several dates of services submitted for financial gain. This is a Criminal Financial Exploitation Md. CRIMINAL LAW Code Ann. § 8-801(a)(1), (8) & (b)(1)-(2) (2022)

The claims are a misrepresentation of the facts. No fee has been awarded nor received. Ms. Davis contacted London Disability on 4/20/22 for representation in her Social Security Disability case. Representation forms were mailed to Ms. Davis on 5/20/22. Ms. Davis signed the representation forms on 5/25/22. The signed forms were received via US Mail by London Disability on 6/1/22. The signed forms and cover letter were submitted to SSA on 6/3/22. The Social Security Administration requires that a claimant sign a second fee agreement when the lawyer attending the hearing is different than the lawyer on the original fee agreement. In this case, we asked Ms. Daivs to sign a second fee agreement bearing my name, her name and the name of the lawyer attending her hearing (Jonathan ******* This is the document Ms. Daivs refused to sign, even though she states in her complaint that she has no objection to the fee requested by Mr. Kopin. Ms. Davis seems to be confusing the original fee agreement that she signed with this second fee agreement that she did not sign. The document Ms. Davis is referring to in her complaint was not signed by Ms. Davis and was NOT submitted to SSA. It appears that Ms. Davis is confusing the document she signed on 5/25/22 with a similar document she didn't sign. The time sheets are accurate. Ms. Davis does not state which entry she is disputing. Due to confidentiality rules there are other facts that London Disability can not disclose to BBB. I left messages for Ms. Davis on 11/4/22, 11/5/22, and 11/7/22. I emailed Ms. Davis on 11/4/22 and 11/7/22. Ms. Davis has not responded to my messages or emails. I can mail the documents that was sent to Judge Posner. See attached Letter Intent to Sue ***** ******. The SSA forms can be confusing and repetitive. I am concerned about Ms. Davis's misuderstanding of the forms. This matter is sooo frustrating, causing so much anxiety. I disagree with the email response from Scott ******. My allegation are still valid. I know that I have cognitive issues now, but to take advantage of a disabled person, Fraud them for financial gain. This is the PROOF..Again, I don't recall receiving anything via mail from ****** Disability between 5/20/22 to 5/25/22. But this is the Email from Scott ****** employee (Kimberely Williams). Please see attached proof ,of an email from Kimberly Williams of ****** Disability, dated 6/1/22@ 3:14pm. (Her statement from the email). Kimberly ******** Social Security Disability and SSI HELP! The forms will allow London Disability to help you with your Social Security Disability/SI claim. I look forward to hearing back from you. Sincerely, Kimberly So based on this statement and emails from other employees (jasmine ******* provides a timeline to CONFIRM that the plaintiff couldn't of signed the Standard FEE agreement, dated 5/25/22 as previously submitted to SSA Judge Posner for Financial Gain. Also, see email proof of the only documents that were sent to your law office. What's hurtful is that Scott ****** is still stating I signed the standard fee agreement, dated 5/25/22. I know that I have cognitive issues now, but to take advantage of a disabled person, Fraud them for financial gain. Sir, this is your last opportunity to straighten out this matter. I know you don't want no public humiliation, this will raise suspicion if you committed this action against another client. Also your staff will have criminal charges against them for fraud, false statements. . If tables were turned an I did this to you ,you would have probably would of filed criminal charges against me, an by me being an African American I would be arrested with out bond for Social Security Disability Fraud, an other charges. Please Scott ****** do the right thing. I'm requesting the following to resolve this matter.. 1., dismiss your standard fee agreement petition in the amount of $6000 your office requested, so I will be able to finally receive my back payment from SSA. You will pay your assistant attorney (Jonathan ******** fees for services 2. A financial settlement agreement in an amount between 75,000 to 100,000 . I will agree not to file criminal charges I will ageee not to file a civil lawsuit , I will agree not to file complaint with the attorney general office I will agree not to file a complaint with the attorney bar association, making a public statement with channel 13 & 45, I will agree not to file complaint with ADA. As noted in the letter of intent to file lawsuit. You have until December 2, 2022 for resolution. Please see attached . As you already know I have more documents. But these are the key documents. Thank you . .. Business Response /* (4000, 16, 2022/11/22) */ Ms. Davis has misrepresented the facts. Ms. Davis has not made any new allegations in her most recent BBB posting. Ms. Davis has confused 2 sets of documents. She signed the first set of documents on 5/25/22 and returned the documents to London Disability via US mail.
