    So my truck was repossessed in 2010 so I got on the bus and walked miles to an Indiana gas station and purchased lottery tickets. I got a *********** game that had multiple drawings on it. So I won ******* dollars I had won before on this same ticket I took it to the Indiana gas station and the cashier told me it was not a winner and keep the ticket. So this time when I won I called the number on the back of the ticket. ************ and I spoke with a male worker I asked if I could check over the phone if I had a winning ticket. I had already seen the drawing and had already seen the winning number and I knew it was a winner. Buy just wanted to make sure. He says yes first I need your name and then he asks for my ticket number. I told him the ticket number and he told me it was a winner. He then asked me for my address and social security number and my driver's license number. Then he told me to get a pen and write my confirmation number my release ticket number and my code to get the ticket. Then he told me to download the claim form and fill it out and send it in with my ticket and a copy of my ID and social security card. So I did just that I got a letter saying I was getting a check for ******* dollars. The thing is I never got the check. I just found the paper that shows all the information about the ticket and the call. I have all the ticket numbers front and back the winning numbers and the dates the ticket was purchased the time I called and spoke with a person. Also the date and time I made the call. So I have been contacting Hoosier Lottery asking about my money I have spoken with an investigator by the name of **** ******* on 12/27/24 and 6/2/25 and 6/6/25. He had me send him a copy of the information I have to his personal phone number telling me he can look into it and get back with me about my money. Also when looking up the winning ticket the next day after the drawing it clearly says there was only one winner for the amount of *******. Ticket signed

    On 9/1/24, I presented a winning Cash Five ticket for the 8/31/24 drawing worth $200 and a free Cash Five ticket to the **************** at *****************************************************. The employee scanned the ticket but said their machine was down and handed me the ticket back.I proceeded to a Speedway at ******************************************* where the attendant scanned the ticket but said he could not cash the ticket and returned the ticket.On 9/3/24, I took the ticket to ****** and Van *** at *********************************. The employee told me the ticket had been previously paid and suggested I immediately go back to the prior location that authenticated it, which was Speedway. Although the manager at ******** admitted a balancing issue on 9/1/24, it was not equal to $200, and they would not pay the prize. The manager suggested I call the Hoosier Lottery.I was directed by Hoosier Lottery customer service to mail the ticket with their claim form. After about 6 weeks the Hoosier Lottery replied with a letter that the ticket was previously paid, offering that I could protest and request an administrative hearing using their online form within 72 hours. I sent the protest next day mail for $30.45.With no response from the Hoosier Lottery for weeks again, I called them. I was informed that my case was reviewed, yet they would still not honor the ticket. The person I talked with did not understand why they refused payment and said she would escalate to her supervisor, although I have again not received a response. She did inform me that it was the **************** at *************************************************************************************** that originally authenticated the ticket. Learning this I mailed Luke *** outlining the situation. With no response from Luke ***, I am filing this complaint. Plain and simple, this is fraud, and the Hoosier Lottery should be dissolved if they refuse to pay due prizes.

    I reported a malfunction in dispensing a $10.00 lotto ticket from the Indianapolis International Airport baggage claim area on lotto machine # ****** on May 17, 2024 @ 13:32 to the contact information on the lotto machine via email. I did arrive on May 17 @ 2:00 AM that morning on my flight when this occurred when no attendants available to report, I had to return later this day for my car rental. This ticket was purchased with cash. Ticket that was attempted to be purchased was called "Indiana Cash Blowout". Immediately after purchase a dispense error was noted with 4 dashes on ticket selection display. Please see attached pictures that were obtained immediately after dispense error was observed.
    I reported to the reporting line noted on Lotto Machine 5-17-24 and a response back 5-18-24 the Hoosier Lotto Investigation Team would look into this dispense issue. This response was from Hoosier Lottery Customer Support.

    The Hoosier Lotto was contacted again on May 31 for an update.
    The Hoosier Lotto was contact again on June 26th with no response.

    They have stole my money and refuse to return my money with documented proof of their machine malfunction.

    This is $10.00 they have stolen from a Hoosier visiting his state during race season. I need your assistance with getting my money back from the Lotto who stole my money and refuses to return my money.

    Deposited $20 in kiosk machine to play my weekly numbers I have stored on their app on me phone. However when I opened thier app to scan my numbers each (7) of my saved gaming numbers they all had notes on the tabs "Game rule change please delete your bet slip numbers and recreate new ones with updated rules. No cash back or change can be received at kiosk. I am a my hoosier member so they have my contact and email however they didn't make contact or reach out to notify members of the change. No notice on kiosk or recent social media messages on change. They took advantage of numerous people. Sure would like someone to hold them accountable.
