************** - June 2019 Removed all upper and lower teeth.Nov 2019 Installed permanent denture stated to last 10 + years, Cost $40,000 paid in full, Cleanings $200. twice a year and X-ray $200. once a year Feb 2020 - First tooth fell out then continued in total 4 teeth fell out (some twice).Nov 2021 - Hygentist cleaned my teeth and created a hole between the reglued teeth. After notifying office, received letter of dismissal.Dec 2021 - ************************************** finally agreed to see me again charged me for an office visit and an x-ray which I paid in advance. When trying to fix the denture, he yelled at his staff, threw his tools and then without fixing he roughly reinstalled the denture (gagging me with chemicals) and told me to leave. For two years I worked with ISDS-** ******************** and the **********************. ****************** attorney offered me $500, they said for release of claim, but ISDS and CDS documented that it was to fix the denture and pay for the office visit and transportation inconvenience. I did not sign the release so I did not get $500. I have been advised that ****************** attorney can drag this through court for many years. I paid $300 to get fixed then again$300 when two more teeth fell out. Now paying to get replaced over $10,000 which is what I asked ************** for if he wanted a total release. I have all documents and dental records showing all fixes and payments. Please me know how to send it to you and I will provide, including pictures of all missing teeth.
