      Dear BBB,I am filing a formal complaint regarding deceptive sales practices by an agent affiliated with First Enroll, who sold me a Federal Advantage+ Priority 100A limited indemnity health insurance policy (underwritten by Federal Life Insurance Company).Key Issues:Misrepresentation of Coverage (November 2023 - June 2024)The agent falsely claimed the policy covered pediatric preventive care including well-child visits and vaccinations for my infant son (Ahan *****, DOB **********)The plan actually excludes these essential services and doesn't meet ACA requirements This critical limitation was never disclosed during enrollment Financial Impact Paid $125 non-refundable enrollment fee Paid $176.01/month (Nov 2023-June 2024) = $1,232.07 Paid $79/month for AD&D add-on = $553 Total Paid: $1,910.07 for unusable coverage Now facing $1,525 hospital bill that was denied ******************************* Provided phone number *************) only leads to automated system System rejects my Member ID and disconnects No response to multiple emails Absolutely no way to get assistance Demands for Resolution:Full refund of all $1,910.07 paid in premiums and fees Reimbursement of $1,525 for the denied hospital bill ******* confirmation that policy has been terminated Assurance that First Enroll will retrain agents to properly disclose coverage limitations Attached Documentation:Policy documents highlighting exclusions Denied claim from ************* Payment records showing all premiums paid Records of unanswered customer service attempts This experience has caused significant financial harm and stress.       I was on state health insurance for 2-3 months. I was repeatedly told there was no wait time for insurance to start AND paper work states no wait for emergency medical apt. I was viscously bit by a dog, (still have scarring) and health insurance will not pay *** ***** Then, believing I was covered and had verification from both *** and insurance, I went in a month later for a sinus infection. Insurance will not cover that either. I have been on the phone with the numerous times. I was unemployed and was billed by both *** and then insurance company for them to pay out nothing! I covered 1 bill but would like insurance to cover the outstanding bill, as they should have.

      I was contacted in may of 2024 by a representative of Federal Life, when i was looking for primary health insurance, i was told it was ********* & needed to find a provider in my area, which i did, in july of 2024, i went in for a routine physical, fortunately i am a healthy person, a couple weeks later, i went to the emergency room for a *******, in august i started recieving bills from both places, in an attemt to figue out what was going on, i cantacted Fedeeal Life to find out why my bills werent being paid, i was told that they were not submitted properly by the healthcare providers & that once done, they would be covered 100%, i had the healthcare providers resubmit in the manner in which i was told for them to do so, then once again the bills got sent back to be unpaid, i contacted the company again, was then informed that my visits were not covered & at that time i learned that my healthcare coverage was NOT actual healthcare coverage but mearly a VERY expensive secondary insurance, like a ******** suppliment plan but i dont even have ********.. the company sent me a whole $50 check to cover the almost $2000 that i now owe because of them.. i am seekimg a FULL refund from the company & in addition to have the bills paid as i was told that they would be, i am also looking into taking further legal action against this company..

      From May - November 2024 we paid monthly premiums of $798 for smarthealth300 plan. We were told this was their top tier plan. However, they paid none of our claims. The claims company said we only had indemnity while the main company said the claims company had the wrong info. After months and hours on hold and being cut off numerous times for being in hold too long, the company was never able to get the claims company to update their paperwork. Finally a supervisor named ******** said they would go ahead and let us cancel and we would receive a refund if all our premiums. He was able to refund the month we had just paid but had to work with his team on the repayment of the other months. We have received nothing. We have called and stayed on hold and left messages with no resolution. In the meantime we paid over $2000 in claims out of pocket and stopped going to doctors when needed due to the expense. We cut back on meds too. This has brought us great stress and financial burden not to mention the toll on our health by stopping seeing doctors during this period. We just want the refund of our premiums that we were promised, and the out of pocket payments we made to cover the services/claims they refused to pay.

      I signed on with Federal Life in August of 2023 and told my doctors visits..diagnostics..bloodwork ...physicals would all be covered..I paid them $203 month or a total of over $3800! All that was paid was a few prescriptions. My doctor that I've had for over 25 yrs tried to file claims to get paid to no avail. I kept getting requests to refile the claims by different **************** reps..All were liars..I had a lung cancer screening in November of 2024 that the diagnostic ctr kept re-billing for...Never got paid. This company turned into First enroll..then Cornerstone..now it's called **************** of America. I was sent $50 on a wellness visit I had in February 2025 and $50 on another visit I had in December 2025. This company is scam artists. One rep said I couldnt drop the insurance by the 7th of the month and pay another premium and she'll nominate my doctor to be enrolled and all bills will be paid...Her phone was disconnected 2 weeks later after I paid another month of premiums!! I want my money back and visits paid for!!

      I bought health insurance for my son, ***************** in May of 2024 and kept it until end of September 2024. I have now received a bill for a doctors visit in June of 2024 of $222.40 that was not paid for by the insurance company. I was told the insurance covered doctors visits, mental health and hospitalizations. They did not pay for mental health visits which I paid for. When I called the drs office, I was told the insurance co sent a check to my son to pay for the visit but we did not receive a check. When calling the claims department, they do not answer the phone. THey say they offer a call back after waiting on hold for 30 minutes but they never call back. I have tried at different times of the day for about 3 weeks. I am wondering if this insurance is a scam. I would like to get in touch with the claims department so they can explain to me why they did not pay the claim and I would like to have the claim paid.       I purchased health insurance from Federal Life Insurance *** for my yearly bloodwork and minor doctor visits. This insurance claimed my doctor was in network and they provide that service. I paid this company $3,285.00 over the 15 months I had their insurance, they paid nothing but $15.00. I now have two medical bills that I just received recently, and one possibly on the way. The total amount of my two bills I already received is $1,179.60, Federal Life only paid $15.00 of that. I want my money back for not providing the service they claimed they would provide.

      Started health care insurance for self and spouse on 4/12/24 for $392monthly We both were sick in Jan 2025 and went to the ***************** said all covered .I got a letter in the mail 3/2/25 saying we owed the full dr **** .Insurance covered $0 and Federal ins sent me a check for $50 said ins cancelled .I called and they said do not cover ***** .I want all my money back for the past 11 months since we were Never even covered !!That is $4312 .I want full restitution .Please Help NCAFE682468548 ****** ********

      This company have been receiving payment for a full year to cover for our deductible as they ended up not paying any of the doctors my wife ******* ****** and I **** ****** went. As a result we are full of bills to pay account going to collection and they dont even answer call nor send an explanation of benefit when you call and get lucky to speak to someone they tell you they have not received bills which have been also sent from us personally.

      I signed up with Federal Life insurance after I retired on 9-11-2023. I was told by the broker,****, who signed me on that this insurance was a private healthcare phcs, multiplan, ppo platinum health,eye & dental. He said no deductible,in network & out of network care.On 9-2-2024 I had to use this health insurance when I suddenly became ill and had to rush to ****************************** emergency room. After being released I was instructed by the emergency room physician to follow up with a gastroenterologist. Upon checking in,this is when we were told by the front desk that this was not a multiplan *************************** paid out of pocket.First I start by calling Westchester billing,who does the billing for ******************* & this is on 12-19-2024. They told me its extremely difficult to reach anyone from Federal Life. The billing ***** did receive a request for medical records from a Anny with **********************. The records were sent but no response.I have since had several conversations with Federal Life who now say they can see the medical records are in the hands of a review team & now waiting on a HIPAA form to be signed by me. No one has ever sent me this form or contacted me from federal Life requesting this. Made several calls to try & retrieve this form from Federal Life, on1-15-2025 I did receive this HIPAA form by email. I signed it & returned it by email.On 1-22-2025 f/u with ************************ rep ***** stated she cant see this ********* takes 15 days to get into my master file to view. Phone back on 1-29-2025 which is the 15th day & speak to a ******* who says that Feb. 5 is the 15th day. Now I dont know what kind of company this so called Federal Life claims to be but they are not a health insurance company that should be in the business of helping people! If this is the way Federal Life does their business they need to be shut down!
