    I completed alcohol dependency treatment at SHARE, located at ****************************************************, phone number ************ and have been trying to get my State required paperwork from them.I was a resident there from approximately 10-3 through *********.I have called and emailed numerous times; with no response or resolution.

    The person filing the complaint requested State Paperwork from us on January 17, 2025.   The State paperwork requested is a unique document that is completed by the clinical team when the paperwork is requested.  This State paperwork was completed by our clinical team on  01/29/2025 and the paperwork was ready for pick up on 1/31/2025.  Ms. ******* was notified on the 31st that paperwork was ready for pick up and paperwork was picked up by her on 2/4/2025.

    Also the Mailing came to Ms. ***** C. *******, Ms. ******* passed away in 2017.  Our new CEO is ******** ******** at the same address.
