My complaint is against the Oak Street ************* located on 63rd and *******. I want to start off by saying that I took my mother to the doctor yesterday on September 27th and when we got there they told us that someone had reached out to her and cancel the appointment she wasn't aware of that and I asked to speak to the manager while we was waiting for the manager the reception is was watching ******* out on the TV which I thought was very unprofessional so we went to the to the waiting room and there was another TV playing loud music the manager came and I asked her was there a private room where we can talk. I was explaining to the manager what had happened about the appointment and I was wondering why my mother has not been seen since May since she's a high risk kidney patient and no blood work has been drawn on her since March of 2024 I was very upset and I also asked her why is the receptionist listening to ******* out and why is the music so loud in a professional setting and she explained to me she will talk to the receptionist and as far as the music the patience request to listen to music I said but do it have to be this loud and by you being a manager you should set the stage not the patience the only thing that should be suitable for a office setting is watching the news or a daytime talk show or something and she really didn't have anything to say so now she said that she's going to let my mother see the doctor so we went back out to the waiting room. And a nurse came in to take her weight and her blood pressure and she gave my mother a flu shot and she said she was going to draw her blood once again another unprofessional setting there was no chair for blood to be taken so I asked the nurse was she a professional phlebotomist and she said no but she know how to draw blood. Once again another unprofessional situation I have more to talk about but unfortunately your system will not allow it.Business Response

My issue started on 9/16/2024,I had injured my foot and needed to see my doctor. I called oak Street health and got the call center which proceeded to put me on hold for some time ,so I chose a call back which never came.Later that day I called back got put on hold again but decided to wait it out. Finally I got to speak to a human who took my information and said my ********* would call me back.This also never happened. Next morning I call again getting call center again explain everything again and wait for a call back. Finally hours later they find the time to call me back.They want me to go get X-rays so I say fine and go to the hospital a couple hours later.When I get to the hospital they don't have an order for X-rays from my doctor . I leave hospital call ** ******** call center again ,explained everything again call center transfers me to referral lady who actually finally faxed the order. Day 3 I go back to hospital and get the X-ray done, go back home wait all day ,no call from *** So I think ok, maby tomorrow. Day 4 still hobbling around on injured foot I call them back ,get call center again ,put on hold again ,finally talk to a human who says they will get a hold of the office and I will get a callback. Well no call back, by now I am beyond ****** I have never had such bad healthcare. Day 5 rinse and repeat. By now it is the weekend so I wait until Monday call again and demand call center transfers me to the ********** I don't want any more call backs. They say they don't have my X-ray results. Ok I call hospital which says they sent them over the same day but will resend and also call ******* I think I will hear something later in the day well- you guessed it nothing. Ok day nine I call get call center. They look up results and tell me the ** will call back by the end of the day, this is at 10 am SURE ENOUGH 4,45. They close at 5.I call the office will call me right back .It is now 6 and no call. I'm dumping this **** poor excuse for a *********Business Response

For the past three weeks I have been hung up on, not given appointments in a timely fashion, cant get an authorization for my medication, my blood sugar levels are all above 140, the one on record is 146 and I have been without medication for two weeks because of the incompetence of someone working at the clinic on ******** in ******* *********. If you do not want to give me the medication for my diabetes, tell me I will go else where so I dont end up dead because of people that do not care and put their feelings before my safety. They have never filled out the paperwork for the authorization and that itself in my opinion someone should be fired for willful disregard to my life.Business Response

I went to ********************************* on ********* in ******* IL on the afternoon of 8/13/24 due to urgent need following 1 week of stomach flu symptoms following a vacation for 2 weeks in the *********** where my symptoms started. I was still sick when I flew in back to *******. I was asked to submit stool and urine samples with my name and date of birth in one bag that the clinic provided which I dropped off on 8/14/2024 at 10 am in the morning but no one has responded to my calls about the results. I have called everyday but no one mentioned anything. I knew it was lost because no one returned my call. I was told that the Quest Lab did not receive any of my specimen samples when called customer service today 8/25/14. So this means that the specimen was lost in their office. I want the office to call me back because of its urgency. I am still sick with same symptoms and may be it should be noted as I got sick out of the country with stomach virus as i never got that so sick in my life. Hoping this clinic abide by CDC guidelines and take seriously on the severity of the situation. Thank you.Business Response

*************, in ********, this facility reach out to myself something like a elderly recruiting this July 2024, I decided to visit this *****, facility what they call introduction to see if this ****** care facility fits your needs everything sounded so wonderful they get all your information / insurance my 1st appointment was July 8 2024, 10:00 am for lab work , next appointment was to be a review of the lab results July 15th 2024, at 9:00 am , well that's when all the confusing started They (Oak St health) just start calling myself over 7x leaving messages to return the call , once you try returning the call nobody at the facility knew who called you next they started canceling my appointment without notifying me until i was already at the appointment this Oak ST health have no communication with myself as a patient they left myself scrambling to find other healthcare on there tv commercial everybody skipping and happy showing great service but it's NOT like that isn't that called FALSE advertisementBusiness Response

*******************, office mgr., Failed to submit bill with ***************** information (UHC not Humana), which I have provided Oak St health in person and over the 800 telephone. ****, Nurse Practitioner Submitted claim with three wrong billing codes (hypertension, heart disease, and another) and excluded the right code for Vitamin Deficiency, causing denial of payment, per DLO phone reps. ******************* failed to schedule me for 6 weeks of onsite counseling with their new social worker, and failed to notify me when she became avail. Per ********* I am allowed to utilize this mental health benefit, which *******************, strongly opposes. *******************, is a very difficult person to deal with, being very rude, hostile, dishonest, argumentative, when you respectfully disagree with her, and she resorts to extreme measures escalating a discussion unnecessarily and creating a hostile, unsafe environment for patients, against Oak Street policy. She is also very incompetent and arrogant. In trying to take care of the billing issue, she kept my bill. She lied about what happened in her office, denied scheduling me for 6 weeks of counseling with social worker which jeopardized my overall mental health and physical well being. Anytime I seek mental health related care from ****, she gets very belligerent, becomes an obstacle, who gets in my way on purpose, for whatever reason. I don't understand it. ********************, in the office gave me horrible medical advice and made inappropriate comments. After telling me that I was Pre-diabetic, he encouraged me to keep eating sweets, telling me "it was unrealistic for me to stop eating sweets cold turkey." I am sorry, but the advise would be to "stop eating sweets" if you're at risk for Diabetes (pre-diabetic) at my age. Also, when I requested a reference to a suitable ********************** for IOP and advised him of bad mental hospital ratings in *****, ******************** rudely Replied, "They aren't *************** Seniors, the Disabled deserve betterBusiness Response

The bill actually was sent to the correct insurance - his claim is actually false. I have tried to recode it despite having to call the local authorities due to his behavior and refusal to leave - unfortunately, the lab has been unable to find a diagnosis code that will cover the lab order - I am happy to get with my account rep and see if she can get it written off if that is what he is wanting.
Also - we have sent him a certified mail with delivery confirmation regarding the termination - he has yet to allow **** to deliverCustomer Answer

I am rejecting this response because: *********************** is being untruthful and escalated bad communication amongst us when I simply asked her to allow me to schedule six counseling sessions with her in her office, which under Oak Street's agreement with the government sponsored ******** and ******** Advantage Plans, I am fully entitled to. The more she said no, the more I asked why, in a very kind voice. That did not stop her from escalating her voice to me as if I were a child. Rewind, a year prior, I was told Oak Street location had a clinical social worker onsite to utilize, but she had quit before I could schedule a visit. When a new social worker was hired, **** decided against scheduling me to see her for personal reasons. **** being a nurse, this goes against even her training because while holding it together in her office, I was emotionally suffering inside and **** knew this and stood in my way. Those six sessions would have helped me. ****, indifferent to my being allowed to question her reasons for the blatant denial, caused her to get upset because she is controlling and wants peopel to accept no without question. That is not who I am. I am a human being who takes charge of his health care and when I asked to speak with the Doctor, who was over her, she started talking loudly, raising her voice and threatening me like those *****'s in videos to call the security to have me removed, simply because I asked her questions. This part is true, that she conveniently left out, in addition to lying that I blocked her in her office and kept her from leaving, when I simply was sitting down in my seat in shock and disbelief that she was acting so unprofessional, and inappropriate for a health care work,w ho showed no concern from my mental well being when she denied those six visits, in addition to blatantly billing the wrong insurance. Her office billed two separate bills for a Janaury 4, 2024 visit. One for a primary care visit, and the other for a Vitamin D defiency blood test. They split the billing up between billing United Healthcare for the doctor visit, and billing Humana for the blood work, for the same date of service. How unbelievable was this? It made no sense from someone who is both a nurse practionier and office manager, who lacks self control, compassion, and the ability to properly bill medical visits. What a horrible organization overall. They also owe me on my AARP membership fees they were supposed to pay, but never did. They are also a business fraud. Please investigate.

Please find attached a spreadsheet with harassing phone calls from representatives of Oak Street Health that I have been receiving since September. 2023.I have filed a complaint on Oak Street Health's corporate website and the ***** website with no response. The couple of representatives that I spoke with were trying to get me to change my primary care provider and my ******************* Please know that I will not change to Oak Street Health due to this continued harassment.Business Response
