On 2/21/23, X-Rays and exams were performed and a credit card approval was done. On 2/27/23, ***************************** and I signed a "Consent for Extraction" document stating that the following eight teeth would be extracted (3, 12, 13, 14, 15, 17, 28, 29). She performed the procedure whereby only four of the eight teeth were extracted. There were three teeth extracted on the upper left side and one tooth extracted on the upper right side of my mouth. No lower extractions were performed. All dental work including extractions, filling, crowns, root canals and lower left side partial was performed by three DDS over a period dating from **** through March-**** prior to my locating to *********, ** in December, ****. Three DDS have performed x-rays, exams and cleanings here in *******. When questioned by hygienists as to how many teeth were extracted, I was told seven, then eight. I am unable to wear upper temporary partial which had to be made twice. I am requesting a refund of four teeth at $334.00 each and a refund of $1119 for the upper partial which I am unable to wear for a total of $2455. The entire amount charged to my Care Credit Account was $5,819.28 on March 9, 2023. No insurance claims are involved. I made an appointment for 10/31/23 and they insist that I need a "deep cleaning" before I receive permanent partials. I was told by office personnel that this deep cleaning could cost up to $2000 which I have left in my Care Credit Account. At this time, I still need dental medical attention and hopefully resolve this issue before moving on.
