    This is the first annual wellness visit as I am looking for a new PCP since Humana cut ties with ****************** and we did not want to change insurance due to husband's cancer. Went and gave blood work in March. 1st. It was an afternoon appt so I did not fast first which caused the cholesterol # to be high. 2nd. I had already told him I did not believe in the lipid test as I was low carb and did not eat sugar/flour. If he insisted on the test, I wanted a test for Vit D3 and an Advanced lipid testing for particle size as the standard test was not accurate. I copied the results off the portal. No where were the requested info. When he came in, I told him I was not there for any reason except an annual wellness exam. I then mentioned the two request. He immediately said that I had not asked for the additional tests. I told him we had talked about it and the request was made. He immediately closed my record and said "we are done" and walked out! He does not have much to say. I feel like he is only looking for $$ and not care. Oh, the previous introduction visit I shared my Lifeline test of clear arteries even though old 1970s science said I had high ldl. He never talked about my good numbers, glucose 85, I use to be hypoglycemic.... Trygly/HDL = 1.7 (more impt than ldl.) If his schooling did not agree, he could have been professional and not lied then walk out!!

    I am a patient of Centerwell Senior Care in ******, ***** *************).I started trying to a medication refill on 2-14-25. As of this morning (2-18-25), I am still trying to get the office to send the prescription to my pharmacy.The have a call center that is no help at ********** **** ***** informed me to start taking 2 tablets a day instead of 1 tablet per day on two of my medications. I am told that there were no notes made on my chart, and then told the doctor had increased the mg instead of the quantity. That is not true. The mg has been 40mg since at least 2023. I can't get an appointment with Dr. ***** until May 2025 to ask her to fix the issue.I have talked to representatives about 15 times so far and they have been of no help either.They location has two medical assistants who have not solved my issue that I am aware of, because I do not have the medications.If Dr. ***** is informed of the issue, I am confident she would take care of it.Still trying to get resolution. I have two medications I cannot get re-filled and am extremely frustrated.

    I am a legally blind and colorblind patient whom suffers from diabetes type 1, lumbar herniated disc that results in excruciating pain in lower back and down my legs, horrible headaches due to a traumatic brain injury, severe depression, and a rare debilitating disease called ******** Syndrome" known to not have a good prognosis.. My Section 8 case worker faxed a form in which it asked if the patient would be in need of a live-in-aide and Dr. ***** claimed that I was not in need of one. I was never called and asked questions about this matter of needing a live-in-aide. I've had a live-in-aide for years without a problem. Every year this form needs to be renewed. My prior doctors understood that I'm disabled with chronic diseases and never hesitated in approving the form for a live-in-aide. This document was needed **** for me to be able to move in a house that I've already paid a deposit for and available to move within a few days. I had given a 60-days notice to vacate from my apartment which has high levels of radon gas which I had told Dr. ***** and the Medical Assistants. They recommended that I should move out and I had started the process. Ofcourse I need a live-in-aide to help take me shopping for groceries and take me to my doctor appointments, help me with cooking and preparing food, help me when my sugar gets too low, remind me to take my insulin, help me avoid falls and eating expired or spoiled foods, etc. It's really sad and very stressfull having to go through this and losing the house I was going to move into. This is destroying my will to live. When I tried to ask her why she did not approve it, she told me that she won't talk about it at all. She had no explanation to why she did not approve it. My 16 yr old daughterr helped me gather some medical records with the hope that Dr. ******* reviews it and approve the need for me to have a live- in-aide. This needs to be faxed to my case worker ASAP so I won't lose the house.

    We have been attempting to work with CenterWell Senior Primary Care (****************************) for the past 3 months or more. I along with the Admissions Coordinator (*******************) from ********************, ** have communicated with CenterWell via phone and fax NUMEROUS times requesting the proper medical records and documentation necessary to have my aunt ********************* admitted to ****** - a full time nursing facility. We have lost a room/space for her once already during the unsuccessful attempts to get the proper and necessary paperwork from her primary care MD at CenterWell. There is a new room/bed available for my aunt, but all necessary paperwork must be received from CenterWell by end of business day, Friday, August 23rd. There are only 2 documents remaining that have not been submitted. A formal order or directive from her MD stating, "Admit to ******************** for long term care, diet and follow current medication list". And *****'s current full medication list. Both of these documents must be signed by her MD (*******************************, MD). ************** called ******************** yesterday on 8/21/24 to inquire what was needed. She left a voicemail message for ******************* - but with no return call back number! In order to reach anyone at CenterWell, you must go through an inefficient call bank of representatives - and request/wait for a call back. Instead of being directed to the individual who can immediately resolve the issue! What we are asking for is easily available to provide IF anyone at CenterWell would make our request a PRIORITY. My aunt's physical and mental well-being and safety is being jeopardized by the lack of urgency and incompetence of CenterWell! Any assistance you can offer will be greatly appreciated. It is extremely frustrating and quite frankly embarrassing to have to deal with them at this point!
