On Dec. 14, 2021 I mailed a NOTARIZED affidavit for Amendment Form **** to correct the death certificate of my great-grandmother, ***********************[s] ******. Since there were no birth certificates when she was born on Feb. 19, ****, I sent all other information that I could to the Vital ************** to PROVE that her parents' names & her husband's name was incorrect on the death certificate I received from ********* **. She died on August 06, ****, so there should not be a problem in finding and correcting this death certificate. I've enclosed ALL those documents, and a copy of the original letter & $40 money order I sent to the *********************** on an attachment, plus a list of the phone calls I have made in attempt to get this corrected death certificate. I need this information on my great-grandmother to file with a Native American nation, so this is a legal matter of great importance. My family and I have already spent over $400 getting all the legal documents and information proving the generational links, and I desperately need ************************* corrected death certificate. The last time Vital Records was contacted they said to give them 30 more days, as they were behind due to Covid problems. I understand that, but it has been about 6 weeks since they said that, and it has been over 3 MONTHS since I mailed in all this information with the request for a corrected death certificate on *********************. I have asked repeatedly that they send this corrected death certificate to me. All I am asking is that they correct her death certificate to show that her parents were ****** ************************************************************** ******. Please help me get this done. ******************** went above and beyond to resolve the problem concerning my great-grandmother's corrected death certificate. She called me to make sure that everything was correct. She apologized for not getting it out sooner. She was extremely kind and thoughtful. I am well pleased with the results. This case is resolved. I tried to comment on Better Business Bureau about this and could not find the case. So, I called them and they erroneously claimed it was not in the system. I would have already corrected this if it had not been for that error on their part. Sincerely, *****************************

I paid $49 to this place thinking I was ordering my birth certificate for my newly adopted Son. I find out this is just a place that fills out paper work. I want to know that my childs information is safe and not compromised. I have reached out to this establishment but I haven't heard back.Business response

Business Response /* (1000, 14, 2021/09/16) */ The customer chose a third party vendor to obtain the birth certificate and not the State Office of Vital Records; therefore, we are unable to refund any of the fees paid to the third party. Our office only receives fees for the actual cost of the certificate(s)and the fee paid for expediting the record, if requested. Customers initiating requests online for vital records should make certain that they are using the official State of Georgia Department of Public Health website and not third party vendors. These vendors do not have access to vital records of customers.
