    * 30 May 2023 I came into dentist for Buildup and Crown * I was asked to pay $1,400 out of pocket if they could not get my insurance company to send them an agreement to pay within 15min of my appointment. * the reason dentist was requesting direct pay from me or agree to pay statement from insurance b/c they stated they have not been paid in past. * ***************** rep confirm dates and amounts of all prior work paid to dentist and office rep confirmed those funds had been paid however dentist still demanded $1,400 for that days' service and I would need to submit claim to insurance company myself * My wife ***************************** had been requesting claim from Dentist since 30 May and they have refused to give us claim to submit to insurance company. Dentist is claiming to have sent to insurance company and conversations with both Met Life and ********* neither of them have received claim and has been requesting it as well.* We believe Dentist over charged us for services that has yet to be completed and he does not want us to inform the insurance company of the over charging.* I believe dentist is trying to take advantage of me being a Deployed US Soldier who cannot go into his office to demand this be fixed. My wife ***************************** has receipt of $1,400 payment and has the authority to act on my behalf.* We are requesting the full claim to submit to insurance company, a check from dentist office for any over payments, and the permanent crown that was never given to us.

    During an unprofessional experience a year ago, I went into the office for a general cleaning and exam for a chipped tooth. The receptionist explained that my appointment had been cancelled. I asked why and she did not have a clear reason why. They were able to get me to look at my tooth but I had to reschedule for the cleaning. She was very rude and just stated that there was nothing she could do to change the situation. I explained to Dr. M**** the conversation that I had with the receptionist but he made up excuses and talked over me.

    On August 19, 2021 I went in for a general cleaning and a young lady, who I had never met before, began cleaning but I had water and saliva running down my cheek and neck. I had to quickly grab the clothe to dry my face.
    I had to react several times to make her stop because she would not address the problem. She was so focused on the cleaning that I had to grab the suction myself to spit. After a horrible experience, she stated that Dr. M**** will come to complete the exam. I'm not sure how long I waited but I had to ask the young lady if he was coming to check on me. She left to notify him that I was waiting and when she came back confirmed that he was in his office.

    I don't know how long I waited but it felt like more than 15 minutes. I eventually got up and went to check out. As I discussed my concerns at check out, Dr. M**** came out of his office. I then went to the front desk to continue my conversation and that is when Dr. M**** abruptly directed me to speak with him. I explained that because he was not available earlier that I was speaking with someone else. He replied, well I'm here now...talk to me, talk to me. I was trying to be discrete because I didn't want the young lady to hear me but Dr. M**** got loud and moved closer in my face. He got loud and I replied to stop and back up. The receptionist apologized and wanted to know if I wanted to make another appointment in which I replied no, that I will never return to that location. I did advise that I was going to file a complaint because this was not my first concern here but it would be the last.
