    RAVE, according to their billing department, owe me $76.61 back to my FSA card and it still has not been refunded for 2 months

    I think this complaint was mis-directed as we cannot locate a patient by this name. Please ask patient for her account # or further details about where she had services. We are located mostly in ********.

    I had 2 different imaging procedures 8/11/22 and 3/17/23. I was quoted a cash self pay for each because my insurance is out of state and high deductible meaning would not cover. When filling out paperwork I did provide insurance information as requested but clarified I would not have coverage for either procedure and was quoted $225 for first procedure and $59 for second. They sent the bill through my insurance anyway and now billing me $342.96 and $74.14 for these procedures instead of the cash price we agreed upon. I have called and spoken with manager ********* who states that because my insurance was provided they billed and now i have to pay the insurance price which is much higher. That they cannot correct this mistake on their part I must take responsibility. But I clearly stated at each appointment I was cash pay...did not know I needed to demand new paperwork without insurance information included. not possible to know this.

    DOS 8/11/23: The patient was informed that if her authorization did not come through for *************, she could choose to be self-pay.
    At time of service her auth was in place, the patient presented her insurance card at check in and paid an estimated responsibility of $171.60.
    Her insurance company has now determined that the remaining balance of $195.36 would be applied to her deductible and is the members responsibility.

    I am rejecting this response because: as originally stated i only provided insurance info because it was requested at time appointment set up.  I was clearly discussed my insurance would not cover either appointment  and i was provided a self pay rate which was much less than what the insurance rate is.  Its not my fault they went ahead and billed my insurance.  I actually had 3 appointments.  The first i successfully paid cash rate.  Not sure why they went to my insurance on 2nd and third.  This is their mistake not mine.  I should not have to pay the additional amount they charge insurance more than self pay.
