    BALANCED BILLING VIOLATION On 4/7/23 I had a routine colonoscopy with ******************************************** (Gastroenterology at Van ****, ************* ********************************, Phone: **************). He is IN-NETWORK.The location was **************************** (**** Van D*** ************************, **************). Its services are IN-NETWORK.Approximately ten (10) minutes before the operation, I was introduced to the Anesthesiologist (**********, MD - BAY AREA ANESTHESIA LLC, ******************************************************************, **************,) who only asked health-related questions. I was NEVER told that she (the Anesthesiologist) was OUT-OF-NETWORK.About a week after 6/11/23, I was presented with a bill for $876.32 from the Anesthesiologist's **************************, Envision (PO Box ****, *************, ** 33075-****) for the services rendered during the operation.I consider this an unlawful act or ommission.I have talked to all three major parties involved in this problem:(A) My primary doctor *** B. Small, gastroenterologist) who performed the procedure/operation.(B) the facility (****************************, the Hospital0 where the procedure/operation took place.(C) the anesthesiologist's office, (BAY AREA ANESTHESIA LLC,) who is responsible for this misconduct.They have all denied responsibility. All, but the anesthesiologist, are IN-NETOWRK. In accordance with the No Surprises Act, I am protected from out-of-network charges when I get care related to a visit to an in-network Hospital outpatient department. I did not sign any sign a notice and consent form that would have allowed the Anesthesiologist to charge me an non-ballanced bill!
