      I was referred to ************** by my Dentist. In conjunction with the dental work I was having done, I needed braces to correct some of my teeth. During the initial consultation ************** stated he would give me a $1000 referral discount on the services. I later learned that the contract I signed was over $6000 (no discount). On that day ************** put on my braces. I paid over $5000 towards these services. The payments were made with ******** Flex Benefits. Now that the benefits have been reduced I am no longer able to make monthly payments on my outstanding $600 plus balance. The payments were made in advance and not $240 monthly as noted in the contract. I will not receive additional flex benefits until 2025. Because I paid in advance and can not make any additional payments I have been denied any further consultations and or office visits. I have not received $5000 worth of services by **************. He and his staff is unwilling to work out a plan. I acted in good faith by paying $5000 within 6 months for services in advance. However, the staff nor Doctor is sympathetic to my situation.I am hoping that you can contact his office and we can resolve this issue. I am forced to walk around with these braces and no medical care. They are sharp and cutting through my gums.

      ***************************** has never been denied treatment at our facility/office. He was last seen in our office 5/15/24. Previous to that, he was seen 4/4/24. He has rescheduled multiple appointments since then on 6/25/24, 7/01/24, 8/05/24, 8/15/24 and 8/26/24 (today). The patient has failed to make a payment towards their account since 2/20/24, I have attached their statement to this email for your reference. If you need any additional information please do not hesitate to reach out to our office manager ****** at anytime.
