    On 06/04/2025, I pulled my credit report and was shocked to find two collection accounts from FL Emer Phys **** Assoc MD. I had never received any bills or notices about these charges.I called their billing department and was told one charge was for an ER visit on 09/26/2022 for $2,314, and the other for a visit on 01/19/2023 for $2,101. These were sent to ****************************, which then passed them to ************************ and Central Portfolio Control.This is deeply concerning for several reasons: I received financial assistance from ************ for both ER visits, and all balances were paid. I was never billed or notified by FL Emer Phys **** Assoc MD before these were sent to collections. Under the No Surprises Act, I should not be balance-billed for out-of-network ER providers without proper disclosure and consent. This action also violates the ****** which requires debt validation and proper notice before reporting to credit bureaus or assigning to collection agencies.I request the following: Immediate removal of these accounts from my credit report. A written explanation and itemized billing. Confirmation that all collection activity will cease.Ive attached documentation of my financial assistance approval and credit report entries. If unresolved, I am prepared to file complaints with the ****, state attorney general, and *** for possible violations of healthcare billing protections.

    Recently I have been receiving calls from ***************** (a debt collector) for services performed by Florida Emergency Phys ************************* on 5-7-23 & 7-5-22. I never received a bill from these Physicians for these charges and hadn't known about them until I received these calls from ********* this year. I have been in discussions with *********, but I feel very uncomfortable paying for these services when I don't know if they are correct. I've tried calling Advent Health to get more information and I was told that emergency physicians bill separately and they wouldn't be aware of the charge. When I try to locate a phone number for Florida Emergency Phys Kang Associates, I get that the office is closed. With no number or address I do not feel comfortable paying for charges when I don't even know what they are for and whether they put through the charges to my insurance company or not. These charges have hit my credit report, and my understanding is they can no longer hit my credit report. Per the ************************************ (Docket # CFPB-2024-0023) these charges should no longer be on a credit report.

    A collections from this company appeared on my credit report for over 1200 dollars. I understand this is supposed to be a billing company, but the only medical debt I owe to anyone is about 60 dollars to advent health respiratory. This needs to be removed ***** my report and this account needs to be canceled immediately.

    I contacted advent health. they said they do not know who you are or what physicians you are affiliated with. My services were comped as charity according to the advent heath **************  I don't owe them anytjhing. Your response is rejected. I have reported this account to the credit bureaus as fraudulent.

    I was checking my credit report and noticed two accounts with the name FL EMER PHYS **** MD. one opened 08/19/2024 and the other just recently 03/12/2025. I'm unaware of what these are for. I haven't even seen a doctor since my last child was born in ***********. And definitely didn't see anyone medical wise in March of this year. So I would like to know what these accounts on my credit report are. Both accounts totals together equal $4811.00 please explain.

    11/09/2022 i was seen at advent health kissimmee for only an xray and left, no doctor saw me. I spoke with Advent Health billing department and told me I do not have any bill or any other bill with there third party or with there doctor and that i was also approve with finacial aid to cover everything after insurance.     I had a collections account added that I believe to be fraudulent or added by mistake. I was seen in the ********************* ER and was insured at the time. I paid my copay in full and should not owe any balance per advent healths billing department. This needs to be removed.

    I was seen at Advent Health (Florida Hospital) on 7/12/2024 emergency department. I provided 2 forms of health insurance Primary and Secondary. The facility visit was covered. Then September of 2024 there was a separate Physicians charge added to my credit profile (Transuion, Equifax, Experian) , for the same date of service 7/12/2024 same emergency room visit in the amount of $1243.00 from a company by the name of Wakefield. I disputed this being that I have never done any business with this company. I received a ***ly back informing me that this charge was for the Physicians fee for the doctor that saw me that day. I called Wakefield and spoke to a *** to confirm both of my health insurances information was on file and to ask why wasn't the claim for this medical visit sent to both health insurance companies. The *** at ********* informed me that she could only see one health insurance on file but she could not tell me which one it was and that I must call my health insurance companies and ask them why the didn't pay the Physican fees to Florida Emergency Physicians and she informed me that Wakefield is an collection agency for them because the Physicians bill was not paid. I called both my health insurance companies and they both stated they did not receive any medicals claims for payment of my visit on 7/12/2024 from Florida Emergency Physicians. One of the ***s from my primary insurance company even took the initiative to call Wakefield and speak to a *** there and was told that they did in fact send a medical claim but this was lie. I did further research only to find that I am not the only person experiencing bogus charges from this company. There are many others and they are not being truthful about claim submission and medical claim amount due and are billing consumers and adding to our credit profiles and giving our information to a collection agency to come after us. I am requesting this account to be removed from my credit profile all 3 bureaus.

    I got a charge of $528 on my credit report. I also got a notification from my bank that my social was found on the dark web. These people are scheming. I called advant they stated my balance is $0 I have health insurance. This place is a scam.
