    Entered into contract on 10/23/24. and paid $8,500 in advance of required treatments. I received treatments for my knees on 10/23, 11/6, 12/4, and quarterly through 6/18. I was supposed to receive a platelet rich plasma (PRP) injection on 9/17. The business appears to be closed. I have left countless phone messages with no response. I am now over a month overdue for my injections. I have called national headquarters at 904-688-9616 and the answ****g service told me that my issue has been elevated. as per, ***** ******* ****** and finally ****. A month has passed without a call back! My knees are beginning to hurt again without the required injections. I am willing to go to another QC Kinetix office for treatment if the Ponte Vedra location is no longer available but still no response.

    This is a screen shot of the $8,500 being debited to my ******* ***** account on 10/24 the first day of the procedure.  The payment will not be in question.  They just closed the Ponte Vedra location before they completed my procedure.  Gainesville office needs to call me and allow me to complete my procedure as guaranteed by the closed Ponte Vedra office.  My mobile is ************.
