              DOCTOR ****** OF SUSSEX PAIN RELIEF CENTER CAUSED INJURY TO MY LEFT FOOT ON 8-202020. THEN CAUSED INJURY TO MY RIGHT KNEE ON1-7-2021. SHE LEFT ME IN MIDDLE BACK PAIN FOR 18 WEES UNTIL SHE FINALLY INJECTED THE VERTABRA THAT WAS MY MAIN COMPLAINT. I FILED A COMPLAINT WITH MEDICARE'S LIVANTA INVESTIGATORS. DR. ****** IMPEDED THE INVESTIGATION BY WITHHOLDING MY CONCENT FORMS FROM THEM AND THEN PROVIDED THEM WITH ALTERED MEDICAL RECONDS. SHE SENT ME A FINAL BILL FOR SERVICES THAT SHE WAS UNABLE TO PROVIDE. AND ALSO BILLED ME FOR A TELEVISIT THAT I CANCELLED. I WOULD LIKE MY EXPERIENCE POSTED ON YOUR WEB SITE TO WARN OTHER PEOPLE ON THE PROBLEMS WITH THIS BUSINESS. I HAVE SPENT OVER $2500.00 IN COPAYS FOR THE KNEE INJURY. IF YOU KNOW HOW TO SET UP ABITRATION, I WOULD APRECIATE THE INFO. I REQUESTED A COPY OF MY CONCENT FORM FOR 2-24-2021 AND NEVER RECIVED IT. IF YOU HAVE A WAY TO GET IT , I WOULD APPRICIATE IT ALSO.

              If I could leave zero stars I would have. I saw Dr Elmati and after I spoke to him about my lower back pain, he pressed his thumb extremely hard into my back. I left his office in pain and still have so much pain I can hardly move. I will never be using this facility!!!. I do not recommend anyone using this facility unless you want to leave with more pain than you walked in with. I think they need to change the name of this facility to just Sussex Pain and remove the “relief “
